摘要
目的建立猪冠状动脉再狭窄模型,并用血管内超声(IVUS)进行评价。方法选择健康小型猪9头,在左前降支中段植入金属裸支架[支架/血管直径=(1.1~1.2)/1.0],术后第28天复查造影、IVUS,并行组织形态学检查观察再狭窄情况。结果4头猪在支架植入2 h内死亡,5头猪完成实验后复查,定量冠脉造影(QCA)示支架植入后血管直径为(2.90±0.26)mm,比植入前基础血管直径[(2.52±0.31)mm]明显增大(P=0.001);随访期血管直径[(1.20±0.77)mm]较植入后血管直径明显减小(P=0.002),直径狭窄百分比为(60.2±23.5)%,支架内再狭窄率为60%。组织形态学检查示管腔面积与支架内面积相比明显减小[(1.29±1.02)mm2vs(6.04±1.21)mm2,P<0.01],平均面积狭窄百分比为(77.9±19.7)%。IVUS检查结果类似[管腔面积(1.67±1.17)mm2vs(5.85±0.94)mm2,P<0.01],平均面积狭窄百分比为(71.0±23.3)%。两种检查手段测得的支架内面积、管腔面积、新生内膜面积和平均面积狭窄百分比相比差异无统计学意义(P>0.05)。结论成功建立猪冠状动脉再狭窄模型,IVUS检查可作为猪冠脉再狭窄模型支架内再狭窄的评价手段。
Objective To develop a porcine restenosis model of coronary artery, and to evaluate it by intravascular ultrasound (IVUS). Methods Nine bare metal stents were implanted into left anterior descendings (LADs) of nine swines [ stent/vessel = (1. 1 -1.2)/1.0]. Then 28 days later,the in-stent restenosis were measured by quantity coronary angiography (QCA) , IVUS and histomorphology respectively. Results After 28 days five swines were undergone re-examination. QCA showed that the post-stenting vessel diameter increased markedly compared with base vessel diameter [ (2.90 ± 0.26)mm vs (2.52 ± 0.31 )mm, P = 0. 001 ]. The fol- low-up vessel diameter [ ( 1.20 ± 0.77 ) mm ] was diminished markedly compared with the post-stenting vessel diameter (P = 0. 002), and the percentage of diameter stenosis was (60.2 ± 23.5 ) %, while the in-stent restenosis rate was 60%. Histomorphometric results showed that the lumen area was diminished remarkably compared with the in-stent areas [ ( 1.29±+ 1.02) mm^2 vs (6.04 ± 1.21 ) mm^2 ,P 〈0.01 ] ,and the average percent area of stenosis was (77.9 ± 19.7 )%. IVUS analysis yielded similar results [ the lumen area ( 1.67 ± 1. 17) mm^2 vs (5.85 ± 0.94) mm^2,P 〈 0.01 ] ; the average percent area of stenosis (71.0 ± 23.3 ) %. Respective area of instent,lumen,neointimal and the average percent area of stenosis acquired by IVUS analysis were consistent with histomorphometric results (P 〉 0.05 ). Conclusions A porcine restenosis model of coronary artery was successfully produced in which in-stent restenosis can be evaluated by IVUS in a porcine restenosis model.
