摘要
【目的】观察中药穴位敷贴疗法对支气管哮喘患者血嗜酸性粒细胞(EOS)、白介素-4(IL-4)及γ干扰素(IFN-γ)等水平的影响。【方法】将66例患者随机分为观察组44例和对照组22例。观察组用胡椒、白芥子、细辛、白芷、半枫荷、姜油、姜汁等,对照组用姜汁配红米、黑米、玉米等,分别制成药饼敷贴穴位。两组均选用大椎、肾俞、肺俞、脾俞(第1组穴)和天突、膻中、气海、关元、足三里(第2组穴)穴位交替进行治疗,每周敷贴2次,共贴8次。观察治疗前后EOS计数,IL-4、IFN-γ水平及肺功能1秒钟用力呼气容积(FEV1)值的变化。【结果】治疗后观察组血EOS计数显著性下降(P<0.01),对照组治疗前后无显著性差异(P>0.05);观察组与对照组治疗后比较,EOS计数变化值有显著性差异(P<0.01)。两组FEV1值治疗前后均无显著性差异(P>0.05)。治疗后观察组血清IL-4、IFN-γ水平有显著性改变(P<0.05),对照组治疗前后无显著性变化(P>0.05),治疗后两组间IL-4、IFN-γ水平比较无显著性差异(P>0.05)。【结论】中药穴位敷贴疗法治疗非急性发作期支气管哮喘,可能通过调节体内细胞免疫反应抑制嗜酸性粒细胞介导的气道炎症性反应,从而减少哮喘复发频次。
[Objective] To observe the effect of acupoint application on serum levels of eosinophils (EOS), interleukin 4 (IL-4) and interferon y (IFN-T) in patients with bronchial asthma. [Methods] Sixty-six patients were randomly divided into two groups: group A ( N = 44) and group B ( N = 22) were treated with alternative acupoint application of medicinal cake on the acupoint group 1 ( Dazhui, Shenshu, Feishu, Pishu ) and acupoint group 2 ( Tiantu, Tanzhong, Qihai, Guanyuan, Zusanli), twice per week, 8 times in total. The medicinal cake for group A was made by ginger juice and ginger oil mixing with herbal powder of black pepper, Semen Sinapis, Herba Asari, Radix Angelicae dahuricae and Radix ptemspermi Hetemphyllum, and that for group B was made by ginger juice mixing with red rice, black rice and corn. The changes of serum EOS, IL-4 and IFN-γ levels as well as the forced expiratory volume in one second (FEV1) were observed before and after treatment. [ Results ] After treatment, blood EOS count decreased markedly (P 〈 0.01 ) in group A and there was no significant difference in group B (P 〉 0.05), but the difference between the two groups was significant ( P 〈 0.01 ). No significant difference of FEV1 was found in the two groups before and after treatment ( P 〉 0.05). Serum IL-4 and IFN-γ levels after treatment differed from those before treatment in group A ( P 〈 0.05) but not in group B ( P 〉 0.05 ). There was no significant difference of IL-4 and IFN-γ levels in the two groups after treatment ( P 〉 0.05). [ Conclusion] Acupoint application shows a therapeutic effect for bronchial asthma in non-acute attack stage by reducing the attack frequency, and its mechanism may be related to the regulation of cellular immune function thus inhibit the airway inflammatory response mediated by EOS.
