彩色多普勒超声评估老年男性高尿酸和高三酰甘油血症患者的血管内皮功能

Assessing the vascular endothelial function of elderly men with hyperuricemia and hypertriglyceridemia by color Doppler ultrasound

摘要 目的:探讨彩色多普勒超声评估老年男性高尿酸和高三酰甘油血症患者血管内皮依赖性舒张功能和非内皮依赖性舒张功能的规律及特点。方法:于2003-10/2006-10选择沈阳军区总医院正常体检人员中无症状男性高尿酸血症和高三酰甘油血症患者各35例。另选32例门诊体检尿酸、血脂正常的健康老年男性为正常对照组。实验经医院伦理委员会审批,受试者均知情同意并自愿参加。实验评估:①采用Olympas全自动生化仪检测血尿酸、三酰甘油浓度。②采用彩色多普勒超声检测肱动脉内皮依赖性舒张功能及硝酸甘油介导的非内皮依赖性舒张功能。内皮依赖性舒张功能:肱动脉内径变化率=(充血状态下肱动脉内径-基础状态下肱动脉内径)/基础状态下肱动脉内径×100%;非内皮依赖性舒张功能:肱动脉内径变化率=(服用硝酸甘油后肱动脉内径-基础状态下肱动脉内径)/基础状态下肱动脉内径×100%;肱动脉血流量(mL/min)=(60×π×肱动脉平均流速×肱动脉内径2)/4。结果:纳入受试者102名,均进入结果分析。①血尿酸、三酰甘油浓度:高尿酸血症组血尿酸浓度高于正常对照组[分别为(510±35),(260±70)μmol/L],差异有显著性意义(t=18.731,P<0.01);高三酰甘油血症组三酰甘油浓度高于正常对照组[分别为(3.2±1.3),(1.3±1.1)mmol/L],差异有显著性意义(t=6.427,P<0.01)。②血管内皮依赖性舒张功能:基础状态下,3组间肱动脉血管内径、肱动脉血流量差异无显著性意义(F=1.292,0.740,P>0.05)。反应性充血状态下,3组患者肱动脉血管扩张、肱动脉血流量增加,3组间差异有显著性意义(F=42.779,26.406,P<0.01),高尿酸血症组肱动脉内径变化率与正常对照组相比有所下降(q=8.841,P<0.05),肱动脉血流量明显低于正常对照组(q=3.786,P<0.05)。高三酰甘油血症组肱动脉内径变化率与高尿酸血症组、正常对照组相比,明显下降(q=4.724,P<0.05),肱动脉 AIM: To evaluate the rule and feature of color Doppler flow imaging in endothelium-dependent dilatation (EDD) and endothelium-independent dilation (EID) of the eldedy men with hyperuricemia (HUA) and hypertdglycefidemia (HTG). METHODS:From October 2003 to October 2006 in this experiment 35 elderly male patients with HUA, 35 elderly male patients with HTG and 32 healthy subjects (control group) were adopted, who all received physical examinations in General Hospital of Shenyang Military Area Command of Chinese PLA. The experiment was approved by the hospital ethnic committee, all the subjects were informed of and agreed to participate in the study.①Olympas automatic biochemical analyzer was used to detect the level of blood uric acid (BUA) and triglyceride (TG).②EDD and nitroglycerin (NTG)-induced EID of brachial artery were measured by color Doppler flow imaging. EDD: changing rate of brachial artery inner diameter (Dd)=(Dd under hyperemia-Dd under base)/Dd under base×100%; EID: changing rate of Dd=(Dd after NTG adminisration-Dd under base)/Dd under base×100%; blood flow of brachial artery (mUmin)=(60×-π×mean flow velocityxDd2)/4. RESULTS:All of 102 subjects were involved in the result analysis.①Compared with control group, the levels of BUA and TG were significantly higher [(510±35), (260±70) μmol/L, t =18.731, P 〈 0.01; (3.2±1.3), (1.3±1.1) mmol/L, t =6.427, P 〈 0.01]. ②EDD: Under base, the change of Dd and blood flow of the brachial artery had no significant difference among three groups (F =1.292, 0.740, P 〉 0.05). Under reactive hyperemia, the vasodUatation and blood flow of brachial artery were both increased, there were significant differences among three groups (F=42.779, 26.406, P 〈 0.01).
