摘要
目的探讨四肢关节内骨折患者围手术期系统康复的效果。方法四肢关节内骨折患者137例,于伤后7~10天实施骨折切开复位内固定手术治疗,从入院后即开始实施系统康复治疗;随机选择116例患者为对照组(术后自行锻炼)。结果治疗组平均随访时间6个月,平均康复疗程(7.2±4.6)周,131例(95.6%)患者关节功能恢复正常,完全恢复伤前工作,6例患者并发关节僵直,无一例因早期康复治疗出现内固定物松动、骨折不愈合、废用性骨质疏松;对照组8例(24.1%)患者术后并发关节僵直须行二次关节松解手术。结论四肢关节内骨折实行围手术期、个性化、系统性康复治疗,能够促进骨折愈合、使伤肢功能障碍降低到最低程度,从而提高了严重创伤患者的生活质量。康复治疗是骨关节损伤治疗过程中必不可少的重要环节。
Objective To study the effect of early perioperative rehabilitation following intro-articular fractures of the limbs. Methods In this study, 137 patients inflicted with intro-articular fractures of limbs were treated with open reduction and internal fixation in our hospital 7 - 10 days after injury. A series of rehabilitation measures were adopted soon after admission. Meanwhile other 116 patients were recruited as control in which the postoperative exercises rely on themselves and may not be mandatory. Results The average duration of follow up was 6 months. The overall course of rehabilitation averaged ( 7. 2 ± 4. 6) weeks. In treatment group, 131 patients (95.6 percent) recovered normal joint function and returned to their former work. Six cases complicated with joint stiffness. No complications such as osteoporosis, malunion or nonunion occurred. In the control group, however, 8 patients (24. 1 percent) had joint stiffness and needed second operation. Conclusion Early rehabilitation could facilitate restoration of the function and range of motion for patients with intro-articular fractures of limbs. The rehabilitation treatment is an essential part in the whole therapeutic process of bone-joint injury and the protocol should be implemented individually according to the patient's condition and the state of the tissues observed during surgery.
