摘要
目的探讨女性不孕症与生殖道支原体感染的关系并对支原体药敏情况进行分析。方法选择804例生育期不孕妇女取宫颈分泌物进行支原体培养及药敏试验。结果804例不孕妇女宫颈分泌物中支原体感染阳性率为51.24%(412/804);其中宫颈糜烂者支原体感染阳性率为58.04%(397/684),宫颈光滑者支原体感染阳性率12.50%(15/120),两者间有显著性差异(P<0.01);804例不孕妇女中,AsAb-IgA(+)者550例,AsAb-IgA(-)者254例。在AsAb-IgA(+)者中有340例支原体感染,阳性率为61.82%(340/550);在AsAb-IgA(-)者中有72例支原体感染,阳性率为28.34%(72/254),两者间有显著性差异(P<0.01);支原体培养药敏结果,交沙霉素、阿奇霉素、美满霉素和强力霉素可作为治疗支原体感染的首选药物。结论不孕妇女支原体感染率很高,且与宫颈糜烂和抗精子抗体密切相关,提示应将支原体检测做为不孕妇女的常规检测项目,并根据药敏试验结果为临床合理用药提供参考,进行规范治疗。
Objective To investigate the infection rate of mycoplasma in infertile women and to explore the relationship between the pathogen and the infertility. Methods The cervical secretions of 804 infertile women were investigated, culture method was used to detect the mycoplasma infection, and then the susceptibility to antibiotics was analyzed. Results The positive rate of mycoplasma in infertility women was 51.24% （412/804）, and the positive rate in women with cervical erosion was 58.04 % （397/684）, whereas in women with normal cervix was only 12.50 % （15/120） （ P〈 0.01 ）. Among the 804 infertility women, the women with positive or negative AsAb - IgA were 550 and 2.54 respectively, and the positive rate of mycoplasma in AsAb- IgA（ ＋ ） women was 61.82 % （340/550）, whereas in AsAb - IgA（ - ） women were 28.34 % （72/2.54） （P 〈 0.01 ）. The mycoplasma susceptibility to antibiotics tests showed that the sensitivity rates of josamycin, azithromycin, minocycline and doxycycline were 65.6% （80/122）, 59.8% （73/122）, 50.8（62/122） and 50% （61/122） respectively. Conclusion The mycoplasma infection rate in infertile female is high and closely related to cervical erosion and AsAb - IgA, so it is very important to eradicate the pathogen. Josamycin, azithromycin, minocycline and doxycycline are recommended to be used in treatment.
