摘要 目的研究乳腺癌围化疗期静脉营养治疗对患者营养状况和细胞免疫功能的影响。方法46例施行乳腺癌改良根治术后行化疗的患者随机分为实验组(24例)和对照组(22例),实验组在化疗前后应用卡文(KabivenPI)作为静脉营养液治疗7d,对照组在化疗前后应用葡萄糖注射液治疗7d,分别测定实验组和对照组化疗前后血前白蛋白、白蛋白、血红蛋白水平,血IgA、IgG、gM、T淋巴细胞亚群CD3、CD4、CD8及CD4/CD8水平,比较上述两组指标。结果实验组患者营养不良状况与化疗前相比未加重,T淋巴细胞亚群数目并不因化疗而减少,反而有所增加,且免疫球蛋白水平在化疗后显著升高;对照组患者营养不良状况与化疗前相比明显加重,T淋巴细胞亚群数目在化疗后有不同程度地减少,且其化疗后免疫球蛋白增加不明显。结论在乳腺癌围化疗期进行营养治疗可有效遏制化疗所致营养不良加重,减少对免疫功能的损害,增加患者对化疗的耐受性。 Objective To explode the effects of nutritional therapy for breast cancer during perichemo stage. Methods Forty - six cases of breast cancer after modified radical operation were randomly divided into two groups. The patients in treatment group (24 cases) were treated with Kabiven PI used as intravenous nutrition liquid for 7 days before and after chemotherapy,while those in control group ( 22 cases) were treated with the injection of glucose for 7 days. PA, ALB, HB, and IgA, IgG, IgM, CD3, CD4, CI)8 and ration of CD4/CD8 were measured and compared between the two groups. Results Compared with that before chemotherapy, the degree of dystrophy in treatment group after chemotherapy was not aggravated, the number of T lymphocyte subgroups was not reduced, and the level of immune globu- lins significantly increased. In control group, the case was quite opposite. Conclusion The nutritional therapy in chemotherapy of breast cancer during perichemo stage can decrease the dystrophy induced by chemotherapy,lessen the damage to immune function and improve the tolerance to chemotherapy.
