摘要
目的采用多对型特异性51物,通过巢式PCR法检测厦门市乙型肝炎病人血清中乙型肝炎病毒(HBV)基因型的分布情况。方法自HBV前S1基因和S基因中区域内设计出10条内外51物,并将其中8条型特异性内引物分成A、B两组,分别扩增A、B、C和D、E、F型HBV,然后将第2轮PCR产物以用3%琼脂糖进行电泳,根据PCR产物片断大小直接判定HBV基因型。应用此法检测了154例厦门市慢性乙型肝炎病人对血清中的HBV基因型,了解厦门HBV基因型分布情况。结果厦门市乙型肝炎病人HBV基因分型结果为B型60例(39.0%)、C型53例(34.4%),B/C混合型41例(26.6%)。HBeAg阳性者中以B基因型占63.3%,B/C型混合感染22.0%(P〈0.005);HBeAb阳性者中以B/C型混合感染63.4%,B型25.0%(P〈0.005)。各基因型在疾病谱分布中无明显差异。结论本研究提示厦门市乙型肝炎病人HBV基因型B、C型者的患病率大致相同;B/C混合感染是一个值得重视的问题。
Objective To investigate the genotypes of hepatitis B virus (HBV) in Xiamen City in southern China by nested PCR with multiplex pairs of genotype-specific primers. Methods Ten outer and inner primers were designed on the basis of nucleotide sequences in regions of the Pre-S 1 and S genes, in which 8 genotype-specific inner primers were divided into 2 groups, mix A and B to amplify HBV of genotype A, B, C or D, E, F, respectively. Electrophoresis of the two groups PCR products of the second-round in a 3% agarose gel was employed to identify the genotypes. Genotypes of HBV were determined directly by the length of PCR products. This method was also used to define the genotype of HBV in 154 HBV infected patients in Xiamen, a seashore city in southern China. Results Of the 154 Hunan patients, 60 (39.0%) were genotype B, 53 (34.4%) were genotype C and 41 (26.6%) were genotype B/C mixed infection. 63.3% genotype B infection and 22.0% B/C mixed infection were detected HBeAg positive (P〈0.005). 63.4% B/C mixed infection and 25.0% genotype B infection were detected anti-HBe positive (P〈0.005). There was no significant difference in distribution of HBV genotypes among the acute HBV infection, chronic HBV infection, liver cirrhosis and hepatocellular carcinoma groups. Conclusion The HBV genotypes prevailed among HBV-infected patients in Xiamen are conf'Lrmed to be types B, C, and B/C mixed infection, and the major genotype is type B (39.0%). Type C infection is also the major genotype among the patients. There is no statistical difference between the clinical groups.
