摘要
目的了解中心性肥胖青少年额叶和海马感兴趣区脑代谢的特征,分析中心性肥胖青少年事件相关电位P300潜伏期与非肥胖者的差异。方法以合肥市某中学初一、初二34名自愿参加的学生为研究对象,通过测量获得身高、体重、腰围、臀围及3处皮脂厚度,计算体重指数(BMI)作为判定肥胖的标准,以腰臀比>同性别年龄组2005年安徽省城市学生腰臀比第90百分位数作为中心性肥胖的界定标准。运用双能X线吸收法(DEXA)测定体成分,磁共振测定脑部海马和额叶部位代谢的特点,事件相关电位测定P300潜伏期。结果中心性肥胖组学生体重、BMI、腰围、腰臀比、3处皮脂厚度、DEXA测定的体脂百分含量及脂肪含量的均数均显著高于非肥胖组。中心性肥胖组额叶部位的氮-乙酰天门冬氨酸(NAA)、NAA与肌酸比值和胆碱类复合物显著低于非肥胖组。协方差分析P300潜伏期结果显示,调整DEXA测定的体脂含量后,中心性肥胖组P300潜伏期的修正均数显著高于非肥胖组,中心性肥胖青少年的P300潜伏期较对照组延长。结论中心性体脂分布可能影响青少年脑代谢过程,并可能对注意力分布和工作记忆负荷产生负性影响。
Objective To understand whether central obese adolescents had neurochemical abnormalities in frontal lobe and hippocampus, and to analyze the change of attention and working memory load distribution by measuring the event-related potential. Methods A total of 34 participants were recruited in a middle school in Hefei. The anthropometric parameters of height, weight, waist circtunference(WC), hip circumference triceps, subscapular and abdominal skinfold thickness(SF) were measured according to standard procedures. The defining of obese was using by body mass index(BMI), and the central obese was defined as waist-hip ratio(WHR) 〉 sex-age-specific P90 cut-off point from data of the Anhui physical timers surveillance of students from urban areas in 2005. Body composition was measured by dual-energy X-ray absorptiometry, and the neurochemical metabolic action in frontal lobe and hippocampus was measured on '^1H-MRS, and P300 latent was measured by event-related potential. Results The means of weight , BMI, WC, WHR, SFs, percent body fat and body fat in central obese group were all higher than those in control group. The contents of NAA, NKA/Cr and Cho on '^1H- MRS reduced in central obese group. In the analysis of covariance, the correct mean of P300 latent period in central obese group was longer than control group after adjusted body fat. Conclusion Body fat in central distribution may influence the metabolism in brain and it can negative affect attention distribution and working memory load.
