替牙过程中正常切牙和磨牙位置的生长改变 被引量:2

Growth Changes of Incisors and Molars Position During Transition

摘要 目的研究替牙期至恒牙初期切牙和第一磨牙牙齿位置的纵向生长变化。方法选择20例(男性6例,女性14例)汉族正常儿童替牙期(9.5±0.9岁)至恒牙初期(13.7±1.3岁)的牙颌模型的纵向资料,通过照相方式获得平行于平面的二维图象,以正中腭平面和第三腭皱近心端作为重替标志,计算机辅助测量切牙和第一恒磨牙各标志点在横向、前后向的纵向变化。结果男性,右侧上、下颌第一磨牙向近中的移动没有统计学意义;左侧上、下颌第一磨牙分别向近中移动2.6mm、3.1mm(P<0.05)。女性,左、右上颌侧切牙唇向移动1.0mm(P<0.01)、1.1mm(P<0.05),左上侧切牙到正中腭平面的距离减少0.45mm(P<0.001)。右侧上、下颌第一磨牙分别向近中移动2.0mm(P<0.001)、2.1mm(P<0.05);左侧上、下颌第一磨牙分别向近中移动1.8mm、2.4mm(P<0.001)。结论替牙期至恒牙初期,相对于正中腭平面和第三腭皱,切牙位置和磨牙横向位置的变化均不明显。上、下颌磨牙明显向近中移动,且下颌磨牙的近中移动量大于上颌磨牙。牙弓深度的减少主要由磨牙前移所致。 Objective The purpose of the study was to investigate the longitudinal growth changes in tooth position of incisors and first molars from mixed dentition to early permanent dentition. Methods Two stages of the dental development of 20 subjects (6 males and 14 females) with normal occlusion were evaluated: stage Ⅰ , when the first molars and four incisors fully erupted; stage Ⅱ , period when the second molars fully erupted. The dental casts were photocopied. Two series of the dental casts were superimposed at the median palatal plane and the median ends of the third palatal rugae, and the changes of the incisors and first molars position were analyzed. Results For the boys, the left first molars in the upper and lower arch mesially moved 2. 6mm, 3. 1 mm(P〈0. 05) respectively. For the girls, the upper lateral incisors on left and right side moved labially 1.0 mm(P〈0. 01), 1.1 mm(P〈0. 05) respectively. The distance of the upper left lateral incisor to median palatal plane decreased 0. 45 mm(P〈0. 001). The right first molars in the upper and lower arch mesially moved 2. 0 mm(P〈0. 001), 2. 1 mm(P〈0. 05) respectively, while the left first molars in the upper and lower arch moved mesially 1. 8 mm, 2. 4 mm(P〈0. 001) respectively. Conclusions During teeth transition, little change wase noted in incisors position and transverse molars position. First molars significantly moved mesially and greater movement occurred in the lower. Mesial movement of the first molars was the main cause of arch depth decrease.
