抑郁症患者的脑电超慢涨落图47例分析 被引量:11

摘要 目的:利用脑电超慢涨落图技术(ET)研究抑郁症患者治疗前后脑内多种神经递质的变化和与脑功能状态的关系。方法:本文47例均符合中国精神疾病分类和诊断标准(CCMD-3)抑郁症诊断标准,于用药前及用药后2、4、6周各作一次ET检测,共4次。采用t检验,分析抑郁症治疗前后神经递质及脑功能状态变化情况。结果:①抑郁症患者治疗前脑内神经递质测值与正常值比较,其γ-氨基丁酸(GABA)、谷氨酸(Glu)、5-羟色氨(5-HT)、乙酰胆碱受体(AchR)、强抑制物质(INH)等测值均高于正常值,而强兴奋物质(EXC)、乙酰胆碱(Ach)、去甲肾上腺素(NE)、多巴胺(DA)的测值均低于正常值,但经配对t检验差异无显著意义(P>0.05);②与治疗前比较,治疗后2、4、6周GABA、G1u均升高,差异有非常显著意义(P<0.01);③治疗后Ach上升(P<0.01);④在治疗后第二周、第四周和第六周5-HT、NE显著升高(P<0.05);⑤汉密尔顿抑郁量表(HAMD)、抑郁自评量表(SDS)治疗前后评分比较差异均有非常显著意义(P<0.01)。结论:抑郁症的发生可能与脑内神经递质GABA、Glu、5-HT、Ach、NE、DA、INH物质、EXC物质和脑功能失调有关。 Objective: By encephaloflugram technology (ET) to observe the change of neurotransmitters in the brain and brain function before and after drug treatment in patients with depression. Methods: the change of neurotransmitters in the brain and brain function was examibed before and the 2^nd, 4^th , 6^th week after drug tretment through ET in 47 patients who accord with Dignostic Criteria for depression of CCMD-3 for 4 times. Results: ①The neurotransmitters such as GABA , Glu, 5- HT, AchR and INH in brain in the patients group were higher than that in the normal group. The other neurotransmitters such as EXC, Ach,NE and DA in the brain in the patient group were lower, which had no significance between the patient group and the normal group (P〉 0. 05); ②As compared with the normal group , both GABA and Glu in the patient group decreased significantly after 6 weeks of treatment(P〈0.01);③Ach in the patient group increased significantly (P〈 0. 01);④ NE and 5-HT in the patients group is decreased significantly from the 2^nd week to 4^th week to the 6^th week(P〈0.05) ;⑤Both HAMD and SDS scores in the patient group decreased significantly after 6^th week of treatment(P〈0.01). Conclusion: Depression may be correlated with the neurotransmitters such as GABA, Glu, 5-HT, Ach, NE, DA, IN, H and EXC in the brain and unbalanced brain function.
