摘要
目的通过对12例慢性铍病患者6年的随访,观察其肺功能的改变。方法选择12例明确诊断为慢性铍病的男性患者,平均年龄为(52±11)岁,进行各项呼吸功能指标的测定。此后每两年随访1次,共随访6年。结果与初次就诊时比较,随访6年中的用力肺活量、第一秒用力呼气容积、第一秒用力呼气容积与用力肺活量百分比、最大通气量、肺活量、肺一氧化碳弥散量、每升肺泡容积肺一氧化碳弥散量和动脉血氧分压均逐年下降,6年后分别降低27.0%、34.5%、18.1%、19.9%、30.7%、37.1%、37.0%和15.5%;残气量、残气量与肺总量百分比、动脉血二氧化碳分压和肺泡气-动脉血氧分压差逐年上升,6年后分别增高88.9%,39.8%、32.4%和43.6%。结论氧化铍明显影响肺功能,尤其是损害肺换气功能。
Objective To study the long-term changes of pulmonary function in 12 patients with chronic beryllium disease(CBD). Methods Twelve male CBD patients, with a median age of (52 ±11) years, were included in this study. Pulmonary function tests were performed every 2 years in patients; the parameters included: forced vital capacity(FVC), forced expiratory volume in one second (FEV1), FVC/FEV1%, maximal ventilatory volume (MVV), vital capacity (VC), residual volume (RV), total lung capacity (TLC), RV/TLC%, diffusion capacity for carbon dioxide of lung (DLCO), DLCO/alveolar volume (DLCO/VA), partial pressure of oxygen (PaO2), and partial pressure of carbon dioxide in arterial blood (PaCO2) ; and alveolar-artery oxygen gradient [P(A-a)O2 ]. Results FVC, FEV1, FEV1/FVC, MVV, VC, DLCO, DLCO/VA, and PaO2 were decreased significantly in all patients. Whereas RV, RV/TLC%, PaCO2 , and P(A-a)O2 were increased significantly. At 6 years of follow-up, FVC, FEV1 , FEV1/FVC% decreased by 27. 0%, 34. 5%, 18. 1%, respectively; and DL.CO decreased by 37. 1%. Conclusion The pulmonary function in CBD patients is obviously damaged by beryllium intoxication, especially for the function of gas exchange. (Shanghai Med J, 2007, 30:326-328)
