摘要
【目的】评估本院及周边医院对严重交通事故创伤患者救治的效果,提出改进措施,提高救治水平。【方法】自2001年到2006年作者使用TRISS方法研究309例严重交通事故创伤患者的预测生存率与实际生存率的情况,通过统计处理,计算Ps值和M检验Z检验,与北美创伤数据库(MTOS)进行比较。【结果】本组病例309例,预测生存200例,实际生存179例,实际生存率57.93%:①穿透伤组存活110例,生存率(83.33%)(M=0.791,Z=2.337),②钝性伤组实际存活69例,生存率(38.98%)(M=0.393,Z=8.937),明显降低。【结论】本组病例总体与MTOS组匹配不佳,结果表明钝性伤组救治水平低于MTOS组,穿透伤组救治水平与MTOS组相当。钝性伤和穿透伤两组比较以穿透伤组匹配度较好,救治水平高于钝性伤组。
[Objective]To evaluate the therapeutic effect on patients with severe roadway traffic traumas (RTT)in Dept of trauma in our hospital. [Methods]The probability of survlval(Ps)was calculated on 309 trauma patients in our trauma section by means of the trauma and injury seyerity score(TRISS)method,the results were compared with the major trauma outcome study(MTOS)by M and Z test. All results were dealt statisti-Cally with the computer statistics system of our hospital. [Results]Among the 309 cases, the calculated expected survivals were 200 and the actual survivals were 179 ones: ① survivals 110 (83.33 %) (M= 0. 791, Z= 2. 337)with penetration injuries ones;②survivals 69 (38.98%) (M= 0. 393, Z= 8. 937)respectively with blunt trauma ones. [Concluslon]Our data are slightly lower than MTOS data in therapeutic level,especially on patients with blunt trauma. However. there may be a little different effect on patients between RTT and other injures. So it's worth doing more study among large samples.
