摘要
目的:总结近年来国内外有关骨肉瘤非手术治疗的研究近况,探讨骨肉瘤非手术治疗的研究进展。资料来源:应用计算机检索PubMed 1980-01/2006-12相关骨肉瘤非手术保肢治疗的文章。检索词“osteosarcoma,limbsalvage surgery”,并限定文章的语言种类为English。资料选择:对资料进行初审,选取包括骨肉瘤非手术保肢治疗的文献,开始查找全文。纳入标准:①关于骨肉瘤非手术治疗的研究。②对具体事件的回顾调查研究。排除标准:重复性研究。资料提炼:共检索到43篇关于骨肉瘤非手术保肢治疗的文献,纳入36篇符合标准的文献:其中9篇关于骨肉瘤非手术治疗的调查研究。资料综合:骨肉瘤是最常见的恶性骨肿瘤,极易转移,有很高的病死率。在骨肉瘤手术治疗中,保肢治疗始终是一个难点。非手术治疗在一定程度上可以在术前缩小骨肉瘤体积,从而获得较为安全的手术切除边缘。为了促进骨肉瘤保肢手术的进步,本文对骨肉瘤的化疗、物理治疗、免疫治疗、基因治疗、介入治疗以及放射治疗相关进展进行综述。结论:骨肉瘤的术前治疗应综合采用各种非手术治疗的方法,从而提高骨肉瘤保肢手术治疗的疗效。
OBJECTIVE: To review the advances in non-surgical operation of osteosarcoma in recent years at home ana abroaa, and explore the research progression of non-surgical operation of osteosarcoma.
DATA SOURCES: The literatures of non-surgical limb salvage therapy of osteosarcoma were collected from the PubMed database between January 1980 and December 2006 with the key words of "osteosarcoma, limb salvage surgery" in English.
STUDY SELECTION: Data were selected primarily. Inclusive criteria: (1)Researohes on non-surgical operation of osteosarcoma. (2)Retrospective researches on specific events. Exclusive criteria: Repetitive studies. Then the related articles were selected to look up full texts.
DATA EXTRACTION: The literatures of non-surgical limb salvage operation of osteosarcoma were 43 papers. Totally 36 papers accorded with the criteria were included, and 9 papers of them were researches on non-surgical operation of osteosarcoma.
DATA SYNTHESIS: Osteosarcoma is the most common malignant bone tumor with high metastasis and mortality. Limb salvage surgery is a difficult point in the operation therapy of osteosarcoma. Non-surgical operation therapy can contract the volume of osteosarcoma before the operation so as to acquire a more safe edge of surgical operation. This paper reviewed the advances in Chemotherapy, Physical Therapy, Immunotherapy, Gene Therapy, Interventional Therapy and Radiotherapy of osteosarcoma so as to improve the limb salvage surgery of osteosarcoma.
CONCLUSION: Various non-surgical operation treatment shall be adopted comprehensively before the surgical operation of osteosarcoma in order to raise the curative effect of the limb salvage surgery.
