持续性心房颤动导管消融前后血浆脑钠肽水平的变化

Changes of plasma brain natriuretic peptide levels before and after catheter ablation in patients with persistent atrial fibrillation

摘要 目的观察持续性心房颤动(简称房颤)导管消融前后血浆脑钠肽(BNP)水平的变化。方法测定68例持续性房颤且无器质性心脏病患者导管消融术前及术后90 d的血浆BNP水平。结果在导管消融后大于3个月的随访中,12例(18%)患者复发心房扑动或房颤。导管消融后3个月血浆BNP水平由术前(80±17)ng/L降为(27±9)ng/L,与术前比较差异有统计学意义(P<0.01)。12例复发房性心律失常患者中,术后血浆BNP为(44±8)ng/L,而维持窦性心律者术后血浆BNP为(25±8)ng/L,两者差异有统计学意义(P<0.01)。结论持续性房颤导管消融后BNP水平下降,且术后维持窦性心律组BNP水平明显低于复发房性心律失常组。 Objective To investigate the changes of plasma brain natriuretic peptide(BNP) levels before and after catheter ablation in patients with persistent atrial fibrillation (AF). Methods Plasma BNP levels were measured before and 90 days after catheter ablation in 68 patients without structural heart disease. Results Exceeding to 3 - month follow - up after catheter ablation, atrial flutter(AFL) or AF recurred in 12 ( 18% ) patients. Compared to that before catheter ablation, Plasma BNP levels were significantly decreased in all the patients from ( 80 ± 17) ng/L to ( 27 ± 9) ng/L after catheter ablation( P 〈 0.01 ). Plasma BNP levels in patients with recurrent AFL or AF were (44± 8) ng/L,however,those were (25 ± 8) ng/L in patients with sinus rhythm, and there was significant difference between the two groups ( P 〈 0.01 ). Conclusion In patients with persistent AF without structural heart disease,plasma BNP levels were decreased and were strikingly lower in patients with sinus rhythm than those in patients with recurrent AFL or AF after catheter ablation.
