摘要
目的探讨抑郁症与帕金森病患者脑多巴胺转运蛋白核素显像的特点。方法采用多巴胺转运蛋白显像剂(^(99)Tc^m-TRODAT-1)通过单光子发射型计算机断层显像(SPECT)成像技术,对13例抑郁症患者、17例帕金森病(PD)患者和10名正常对照进行脑多巴胺转运蛋白分布核素显像。结果三组比较,右侧纹状体与左侧纹状体(RST/LST)比值比较,差异无统计学意义(P>0.05),而右侧纹状体与小脑(RST/CB)和左侧纹状体与小脑(LST/CB)放射性比值的差异有统计学意义(均P<0.01)。其中对照组 RST/CB 和 LST/CB 最高(分别为3.486±0.207和3.553±0.158),其次为抑郁症组(分别为2.255±0.290和2.317±0.341),而 PD 组最低(分别为1.522±0.237和1.547±0.262)。结论抑郁症和 PD 患者的双侧纹状体及小脑的放射性比值均低,其中 PD 患者更显著。
Objective To study the tissue structure in the striatum of brain dopamine transporter sites in patients with major depression or Parkinson disease (PD) and in healthy subjects. Methods Single photon emission computed tomographic (SPECT) brain scans were acquired from 13 major depressed patients, 17 PD patients and 10 health volunteers by using ^99Tcm-TRODAT-1 as imaging agent. Specific regions of interest in the basal ganglia were examined. Specific uptake values of dopamine transporter 99Tcm- TRODAT-1 binding affinity were calculated from the SPECT scan data, and the values between the patients and healthy subjects were compared. Results No significant difference in the radioactivity ratio of right-to- left striatum (RST/LST, means + s) was found between the healthy controls, depressed patients and PD patients ( P 〉 0. 05 ). The radioactivity ratios of right striatum to cerebellum (RST/CB) and left striatum to cerebellum (LST/CB) were ( 1. 522 ± 0. 237 ) and ( 1. 547 ± 0. 262) in PD patients, (2. 255 ± 0. 290) and (2. 317 ± 0. 341 ) in depressed patients, and (3. 486 ± 0. 207 ) and ( 3. 553 ± 0. 158 ) in healthy controls, and the differences were statistically significant. Conclusion The results indicate that decreased activity in the nigrostriatal dopaminergic pathway be in depressed patients and PD patients.
