摘要 目的:概述干细胞、肿瘤干细胞与肿瘤发生发展之间的关系,为肿瘤的研究及临床治疗提供参考。资料来源:检索PubMed1997-01/2007-02与干细胞和肿瘤干细胞相关的文献,检索词“stem cell,cancer stem cell,cancer,development,self-renewal,tumorigenesis”,并限定语言种类为英文。资料选择:对资料进行初审,纳入标准:①肿瘤干细胞目前研究的进展。②肿瘤干细胞在肿瘤治疗中的应用。排除标准:①文献中重复研究和综述文章。②肿瘤干细胞以外的文章。③Meta分析类文章。资料提炼:共收集到52篇关于干细胞治疗及研究现状的文章,通过查找全文重点引用30篇文章。资料综合:目前肿瘤的治疗方法主要是针对肿瘤组织内的大多数细胞,常有复发和转移,使治疗失败。随着白血病、乳腺癌及脑肿瘤的干细胞的分离纯化,肿瘤干细胞学说逐渐成熟起来。肿瘤细胞来源于肿瘤干细胞,是肿瘤复发和转移的根源,可靶向性杀伤肿瘤干细胞从而使根治肿瘤和防止肿瘤复发和转移成为可能。结论:只有小部分肿瘤干细胞才有成瘤及维持恶性显型的作用,因此肿瘤的治疗关键应是针对肿瘤干细胞的治疗,肿瘤干细胞的起源、研究方法及研究内容均需进一步研究探讨,而实际上如果肿瘤是源于肿瘤干细胞,先前诸多关于肿瘤发生发展的机制、细胞信号途径等的研究成果需要重新评价,对传统的肿瘤治疗方式提出了巨大的挑战。 OBJECTIVE: To review the relationship among the stem cell, cancer stem cell and cancer, and provide evidences for the study and clinical treatment of cancer. DATA SOURCES: The articles about stem cell and cancer stem cell between January 1997 and February 2007 were searched in PubMed database by inputting the key words "stem cell, cancer stem cell, cancer, development, self-renewal, tumorigenesis" into computer and the language was limited to English. STUDY SELECTION: The article were primarily checked according to the inclusion criteria: ①the current studies on cancer stem cell, ②the updated progress of cancer stem cell in cancer therapy; and the exclusion criteria: ①repeated studies and reviews,②studies beyond cancer stem cell,③Meta analytic studies. DATA EXTRACTION: Totally 52 literatures of stem cell therapy and current study were collected, among which 30 accorded inclusion criteria. DATA SYNTHESIS: Now the therapy of cancer mainly aims at the majority of the cells within cancer tissue. As a result, recurrence and metastasis often develop, which cause the treatment failure. So far, the cancer stem cells have already been separated and determined in leukemia, breast cancer, central nervous system cancer, the theory of the cancer stem cell is growing. The cancer cells origins cancer stem cell, which can cause recurrence and metastasis, makes it possible to treat cancer radically and prevent it from relapsing and metastasizing specifically killing targeted cancer stem cells. CONCLUSION: Only partly cancer stem cells have an impact on tumorigenesis and malignant phaenotype maintenance, thus the cancer therapy should focus on the treatment of cancer stem cell. Further investigations are called on in the aspects of cancer stem cell source, research method and research content. And some old concepts such as tumorigenesis mechanism, cell signaling pathways etc may need to be reevaluated, that raised great challenge to traditional oncotherapy.
