摘要
目的探讨通过联合应用皮瓣延迟术及术前抽水减张的方法,以增加扩张皮瓣成活的长宽比例。方法对22例应用超出常规长宽比例(常规长宽比为1:1)或扭转角度过大的扩张皮瓣进行的整形修复术者,在扩张器注水成功后,行2次皮瓣延迟术。扩张皮瓣最大面积为32cm×9cm,长宽比例为3.5:1.0;最小面积为21cm×7cm,长宽比例为3:1。于行第1次延迟术前24h释放注水量的10%减压。结果22例术后皮瓣未见动脉血供和静脉回流障碍,皮瓣全部成活,术后效果满意。结论联合应用皮瓣延迟术及术前适当减张能明显改善扩张皮瓣微循环,提高扩张皮瓣长宽比例的成活面积。
Objective To introduce a new method of improving microcirculation and extending the survival area of the expanded flap by combining modified operation of the delaying with decreased tension. Method After the skin flap was expanded, the tension of expanded flap was decreased by extracting partly allusion of expander before the first flap delaying operation performed, The skin and the subcutaneous tissue superficial to the expander were incised on both sides along the axis of the flap. After 7 days, the second flap delaying operation was performed. One end of the pedicles was divided so that a pediculated super-long randomized flap was transfered. The flap was transfered to cover the wound on the recipient area. The wound on the donor site was closed directly, Results From 2000 to 2005, this technique was used in 22 patients (13 male, 9 female). The flaps were completely survived after operations. Conclusions The technique of combining modified operation of the delaying with decreased tension can improve microcirculation and enlarge the survival area of the expanded flap.
