代谢综合征患者血清内脂素、脂联素水平的变化及影响因素 被引量:8

Visfatin and adiponectin levels in metabolic syndrome patients and their influencing factors

摘要 目的了解代谢综合征患者(MS)血清内脂素和脂联素水平的变化,并探讨其与MS各组份的相互影响。方法按1999年中华医学会标准筛选MS组42例,健康对照组30例,检测人体测量参数和生化指标,并测定空腹血糖(FPG)、胰岛素(FINS)、脂联素、内脂素等水平。用酶联免疫法(EIA法)测定内脂素,用放免法(RIA)测定脂联素和胰岛素。用最小稳态模型计算胰岛素抵抗指数(HOMA-IR),公式为FPGFINS/22.5。结果MS组的血清内脂素水平明显高于对照组(97.57±40.69)ng/mLvs(50.54±19.14)ng/mL,P<0.05,而脂联素水平明显降低(7.98±3.62)μg/mLvs(13.55±5.72)μg/mL,P<0.01。MS患者血清内脂素水平与腰围呈显著正相关(r=0.415,P<0.01),而与其他代谢指标无明显相关性;脂联素水平与HDL-C呈显著正相关(r=0.579,P<0.01),而与HOMA-IR(r=-0.601,P<0.01)、WHR(r=-0.531,P<0.01)呈显著负相关。结论MS患者的内脂素水平升高,而脂联素水平下降。 Purpose This research is to investigate the change of adiponecin and visfatin levels in patients with MS,and their relation with the components of MS. Methods Based on standards of Chinese Medical Association in 1999,42 MS patients were selected as MS group, and 30 healthy subjects as control group. After measured the heigth, weight and waist circumference,the fasting blood of MS group and control goup were collected and the fasting levels of glucose, insulin, adiponectin and visfatin were determined. The visfatin was measured with enzyme-linked immunoassay (EIA), and the adiponectin levels with radioimmunoassy (RIA). The HOMA-IR was calculated based on FPG and FINS with the minimal model. The statistical data were analyzed by SPSS 10.0 software. Results The visfatin levels of MS group increased significantly than control group [(97.57 ± 40. 69) ng/mL vs (50. 54 ± 19. 14) ng/mL, P〈0. 05], and the adiponectin levels was lower significantly than healthy subjects [(7.98 ± 3.62) μg/mL vs (13.55 ± 5.72) μg/mL,P〈0. 01]. The multiple logistic regression anlysis showed that the visfatin levels was positively related to waist circumference(r = 0. 415, P〈0.01 )of MS patients, but there was no correlation between the visfatin level and other metabolic indexes such as adiponectin, FINS, BMI, HOMA-IR, or triglycerol. There was positive correlation between adiponectin level and HDL-C(r = 0. 579, P〈0. 01 ), but it was negatively correlated with HOMA-IR (r= - 0. 601 ,P〈0.01)and waist-hip ratio(WHR) (r = - 0. 531 ,P〈0.01)in these MS patients. Conclusions The visfatin level increased, but the adiponectin levels decreased in MS patients.
