摘要
目的探索乌司他丁(UTI)在肺缺血再灌注损伤中对肺的保护作用。方法18只实验用犬随机分成对照组、抑肽酶组(抑肽酶30000 U/kg)和UTI组(UTI20000 U/kg),每组6只。建立在体肺缺血再灌注模型,并检测血清丙二醛(MDA)、过氧化物歧化酶(SOD)及肺组织中的MDA、髓过氧化物酶(MPO)。进行再灌注肺肺泡灌洗液(BALF)白细胞计数和肺组织病理切片并计算肺湿干重比(W/DR)。结果再灌注后UTI组动脉血和肺组织中MDA水平、MPO活性较抑肽酶组及对照组低(P<0.05),血清SOD活性较抑肽酶组及对照组高(P<0.05和P<0.01)。再灌注肺BALF白细胞计数UTI组低于对照组(P<0.05);肺W/DR UTI组低于抑肽酶组和对照组(P<0.01)。病理切片显示,UTI组肺损伤比抑肽酶组和对照组轻。结论UTI20000 U/kg可减轻犬肺缺血再灌注损伤,对犬肺的保护作用优于抑肽酶30000 U/kg。
Objective To investigate the effect of ulinastatin on lung ischemia-reperfusion injury in dog. Methods Eighteen healthy experimental dogs were randomly divided into three groups with 6 animals each., control group, aprotinin group (aprotinin 30 000 U/kg), and UTI group (ulinastatin 20000U/kg). Plasma malondialdehyde (MDA),superoxide dismutase (SOD) levels, leucocytes count of bronchoalveolar lavage fluid (BALF), MDA and myeloperoxidase (MPO) levels in lung tissue were determined. Lung water was assessed and histological evaluation was made. Results The levels of MDA,MPO in sera of venous blood and lung tissue in UTI group were significantly lower than those in aprotinin group and control group(P〈0.05). And on the contrary, the levels of sera SOD in UTI group were higher than those in aprotinin group and control group (P〈0.05 and P〈0.01, respectively). Leucocytes count of alveolus lavage in UTI group was less than that in control group (P〈0.05) ,and lung water in UTI group was less than that in aprotinin group and control group (P〈0.01). Conclusion Ulinastatin 20 000 U/kg can attenuate lung ischemia-reperfusion injury, which is better than aprotinin 30 000 U/kg.
