摘要
目的评价持续肾脏替代疗法(CRRT)在脓毒性休克并多器官功能不全综合征(MODS)中的治疗作用。方法应用CRRT治疗脓毒性休克并MODS患儿16例,评价CRRT治疗后12、36 h血压、心率、射血分数(EF)、C反应蛋白(CRP)、pH、氧合指数[Pa(O2)/FiO2]、钠、钾、BUN、Cr、IL-8的变化。结果治疗后12、36 h血压、心率、EF值、pH、Pa(O2)/FiO2、CRP、钠、钾、BUN、Cr、IL-8与治疗前比较差异均有显著性意义(Pa<0.05)。治疗后36 hPa(O2)/FiO2、IL-8、CRP、钠、BUN、Cr与治疗12 h比较,差异均有显著性(Pa<0.05)。余指标亦较治疗12 h改善,但差异无显著性(Pa>0.05)。结论CRRT可改善儿童脓毒性休克并MODS的脏器功能,改善水、电解质、酸碱平衡,为脓毒性休克并MODS治疗提供又一措施。
Objective To study the effect of continuous renal replacement therapy(CRRT) on septic shock complicated with multiple organ dysfunction syndrome(MODS). Methods Sixteen cases with septic shock and MODS were treated by CRRT. Blood presure, heart rate, ejection fraction (EF) , C - react protein (CRP) , pH, Pa ( O2 )/FiO2, Na^+ , K^+ , BUN, Cr, interleukin - 8 ( IL - 8 ) were determined before CRRT and 12,36 h after CRRT. Results Blood presure,heart rate,EF,pH,Pa(O2 )/FiO2 ,CRP,Na^+ ,K^+ ,BUN,Cr,IL -8 in children with MODS before CRRT were significantly different compared with those before CRRT and 12,36 h after CRRT( Pa 〈 0.05 ), And Pa(O2)/FiO2, IL - 8, CRP, Na^+ , BUN, Cr were significantly different from those of 12 h after CRRT ( Pa 〈 0, 05 ) ; the others had no satistical significance ( Pa 〉 0.05 ). Conclusions CRRT may improve organ function, balance of water, electrolyte,acid - alkaii in septic shock and MODS. CRRT is an useful tool in the treatment of septic shock and MODS.
