摘要
目的探讨椎管内畸胎瘤的诊断、治疗,以提高对本病的认识,选择最佳治疗方案。方法对10例椎管内畸胎瘤的临床资料进行分析。结果10例均经手术及病理证实,其中7例为成熟畸胎瘤,2例为未成熟畸胎瘤,1例畸胎瘤恶变。其中7例位于髓外硬脊膜下,3例位于髓内。在影像学上具有特征性表现。手术治疗10例,8例症状消失,2例症状无明显改善,6例回访8个月至4 a,其中4例复发。结论椎管内畸胎瘤多位于髓外硬脊膜下,少数位于髓内,在影像学上具有特征性表现。手术效果好,未成熟畸胎瘤及畸胎瘤恶变者术后多复发,并可有髓内种植转移,故应术后配合放疗及化疗并定期复查CT或MR I。成熟畸胎瘤手术全切后,复发率极低。
Objective To explore diagnosis and treatment of intraspinal teratoma and select the optimized treatment project.Methods Clinical data of 10 cases of intraspinal teratoma was analyzed.Results Ten patients with intraspinal teratomas were confirmed by operation and pathology,7 cases were mature teratoma,2 cases were immature teratoma and 1 case was mutant of teratoma.Three cases were located in subdural space of the spine and 3 cases were intramedullary teratoma.They showed characteristic signification in imaging.Ten patients were performed with operation.Symptom disappeared in 8 patients and no obvious improvement in symptom in 2 patients.Six patients were followed up for 8 months to 4 years and 4 cases were having been achieved have better effect.Two patients showed partial effectiveness.Six patients had been visited from 8 months to 4 years,4 patients were recurrence.Conclusion The major intraspinal teratoma is located in the subdural space of the spine,minority is intramedullary teratoma and they demonstrate characteristic signification in imaging.Operation is effective and immature teratoma and aggressive teratoma indicate recurrence and intramedullary implantation metastasis.Combination of radiotherapy and(or) chemotherapy and regular reexamination of CT and MRI.There is rare recurrence in mature teratoma after complete resection.
