外周血Th1/Th2淋巴细胞失衡与2型糖尿病肾病的关系 被引量:10

摘要 目的通过检测2型糖尿病肾病患者外周血CD4^+辅助淋巴细胞分泌的细胞因子,探讨外周血Th1/Th2淋巴细胞失衡与2型糖尿病肾病的关系。方法运用三色荧光标记法流式细胞术检测外周血中Th1/Th2淋巴细胞。结果2型糖尿病无肾病者Th1数、Th1/Th2比值与正常对照组相比明显降低(P<0.01),2型糖尿病早期肾病患者Th1数、Th1/Th2比值明显高于正常对照组(P<0.01),2型糖尿病临床肾病者Th1数,Th1/Th2比值较正常对照组降低,但无显著差异。2型糖尿病各组Th2数与正常对照组无明显差异。2型糖尿病患者外周血Th1淋巴细胞和HbA1c呈负相关关系(r=-0.688,P<0.01)。结论2型糖尿病患者存在Th1/Th2淋巴细胞的失衡,在其肾脏病变发病的不同阶段,存在不同的Th1和Th2优劣势,细胞和体液免疫在肾病发生的不同阶段发挥了不同的作用。 Objective To investigate the relationship between Th1/Th2 lymphocyte imbalance in peripheral blood and type 2 diabetic nephropathy. Methods The distributions of Th1/Th2 cells were determined by 3-colored fluorescence labeled flow cytometry in diabetic nephropathy (DN) groups and the controls. Results Thl cells and Th1/Th2 ratios were significantly reduced in diabetic patients when compared with the controls (P 〈0.05). Thl cells and Th1/Th2 ratios were significantly increased in type 2 diabetic patients with microalbuminuria when compared with normal controls (P 〈0.01). There were no difference in the Th1/Th2 ratios and Thl cells between diabetic patients with macroalbuminuria and the controls. Thl cells were negatively correlated with HbA1c (r = -0.688, P 〈0.01). Conclusion The results show that Th1/Th2 imbalance exists in peripheral blood of patients with type 2 diabetic nephropathy. Th cells may play an important role in the occurrence and development of DN.
