乙型肝炎病毒免疫学标志物与HBV-DNA的定量分析

摘要 目的:探讨乙型肝炎病毒(HBV)二对半抗原抗体系统、前S1蛋白(Pre-S1)和HBV脱氧核糖核酸(HBV-DNA)之间的关系。方法:用酶联免疫吸附试验(ELISA)法检测209份HBV感染者血清的HBV二对半抗原抗体系统,实时荧光定量聚合酶链反应(FQ-PCR)检测HBV DNA。结果:HBV-DNA滴度小于每毫升1×105cop ies者35份(16.7%),HBV-DNA滴度在1×105-1×107cop ies者88份(42.1%),HBV DNA滴度大于1×107cop ies者86份(41.1%)。HBV-DNA滴度与HBeAg阳性率、Pre-S1阳性率呈正相关,相关系数分别为0.998(P=0.039)和0.882(P=0.024)。结论:HBeAg和Pre-S1也是反应HBV复制的指标。对HBeAg阴性的慢性乙型肝炎患者检测Pre-S1,可帮助判断HBV复制情况。 Objective: To explore the internal relations between expressional patterns of serological markers of hepatitis B virus (HBV) and HBV deoxyribonucleic acid (DNA). Methods: Immunological markers and HBV DNA of 209 serum samples from HBV infected people were detected by using ELISA and real-time fluorescence quantitative polymerase chain reaction (FQ-PCR). Results: Among the 209 samples, 35 (16.7%) had HBV-DNA titres lower than 1 ×10^5 copies/ml, 88 (42.1%) had the titre of 1 ×10^5 - 1 ×10^7 copies/ml, and 86 (41.1%) had the titre higher than 1 ×10^7 copies/ml respectively. HBV-DNA titer was positively correlated with positive rate of HBeAg and positive rate of HBV pre-S1 protein ( Pre-S1 ). The correlation coefficients were 0. 998 ( P = 0. 039 ) and 0. 882 ( P = 0. 024) respectively. Conclusions: Both HBeAg and Pre-S1 are indexes of the replication of HBV to a certain extent. In chronic hepatitis B cases with negative HbeAg, test of Pre-S1 can help to judge the replication of HBV.
