摘要
目的观察罗格列酮干预前后2型糖尿病患者血清瘦素水平的变化,探讨罗格列酮改善胰岛素敏感性的可能途径。方法30例新诊断的2型糖尿病患者随机分为罗格列酮组(4 mg/d)、二甲双胍组(750 mg/d)和磺脲组(达美康160 mg/d)。应用放射免疫法检测治疗前及治疗12周后的血清瘦素水平,同时检测血胰岛素和糖化血红蛋白的水平。结果药物干预前后三组的空腹血糖、糖化血红蛋白明显降低,而空腹胰岛素水平未见明显变化。罗格列酮干预后血清瘦素明显减低,胰岛素敏感性显著提高,而二甲双胍组和磺脲组未见明显变化。结论罗格列酮可能通过降低2型糖尿病患者血清瘦素的水平而改善胰岛素的敏感性。
Objective To observe leptin level before and after rosiglitazone administration and to explore the possible mechanism of rosiglitazone's improving insulin sensitivity. Methods A total of 30 patients newly diagnosed with type 2 diabetes were separated into three groups at random, and treated with rosiglitazone (4 mg/d), metform (750 mg/d) and gliclazide (160 mg/d), respectively. Serum leptin, insulin and GHbA1c were measured after 12 weeks. Results After 12 weeks' treatment, the fasting glucose and GHbAlc in each group declined sharply, while free insulin did not change. The leptin level in rosiglitazone group decreased and insulin sensitivity was improved after administration, but there was no change in metformin and sulfonylureas groups. Conclusion Leptin may play a role in rosiglitazone's mediating the insulin- sensitizing effects.
