摘要
目的探讨侧胸壁皮瓣修复功能部位巨大软组织缺损的可行性及效果。方法对18例功能部位深度烧伤后巨大组织缺损患者采用侧胸壁皮瓣一期修复。切取皮瓣面积16cm×30cm~22cm×30cm,其中游离吻合血管皮瓣16例、岛状瓣2例,结果均一次性修复成功。随访6~12个月,皮瓣稍显臃肿,无需修整。结论侧胸壁皮瓣血管恒定、口径粗大、无变异,供瓣部位隐蔽,血运丰富,切取范围大,抗感染能力强,适用于深度烧伤后软组织缺损的修复。
[Objective] To explore the operation opportunity, method and clinical effect of repairing tremendous soft tissue defects on functional sites with lateral thoracic skin flaps after deep burn. [Methods] 18 burnt patients with tremendous deep burn wounds on functional sites of extremities were selected. Proper debridement was taken and survival tissue was preserved. Then lateral thoracic .skin flaps were applied to repair the wounds primarily, of them 16 were free skin flaps and 2 were island skin flaps. The flap area ranged from 16 cm×30 cm to 22 cm×30 cm. The donor sites were covered by intermediate thickness free skin graft. [Results] The skin flaps survived completely. In one case the vein congested at the distal end of the skin flap. After blood-letting therapy and hyperbaric oxygen therapy, the skin flap survived. The wounds healed by first intention. The function as well as the length of injured limbs was maintained. After the follow-up of 7 cases for 6 to 12 months, the skin flaps looked a little bulky but didn't need to repair by plastic method. [Conclusion] Lateral thoracic skin flap has advantage of reliable vascular support and high survival rate, so it's a good choice for repairing tremendous soft tissue defect on functional site after deep burn.
