异丙酚合用芬太尼用于胃镜检查时病人的循环系统及血氧饱和度变化

摘要 目的探讨异丙酚合用芬太尼用于胃镜检查时循环系统及血氧变化。方法48例接受异丙酚合用芬太尼用于胃镜检查的病人(A组)。另外48例基本情况相似的病人为对照组(B组)。两组病人监测血压(BP)、心率(HR)、血氧饱和度(SpO2)和呼吸频率(RR)。记录操作结束至早期恢复(睁眼、定向力、作指令动作)的时间间隔。结果术中两组病人的平均动脉压(MAP)、脉搏(HR)、血氧饱和度(SpO2)和呼吸频率(RR)比较差异有显著性(P<0.05)。A组流涎、恶心、呕吐等不良反应明显少于B组。BP、HR、SpO2变化显示,检查过程中B组BP均明显升高,升高幅度>20%约18例,而A组BP均有不同下降,降幅>20%有12例,有显著差异(P<0.05)。A组检查中SpO2下降,明显低于B组(P<0.01)。结论异丙酚合用酚太尼下行胃镜检查,反应少,术中循环稳定,减少了操作时间和并发症。 Objective To discuss changes of cardiovascular system and blood oxygen content when propofol and fentanyl were used in combination for patients' gastroscopic examination. Methods There were 48 cases using propofol and fentanyl in combination for gastroscopic examination (Group A), and another 48 cases with the same conditions without using propofol and fentanyl for control group (Group B). In both groups,BP,HR,SpO2,and RR were monltored.And the duration from operation completion to the early resurgent time (oculoreaction, orientation and instructing movement) were recorded.Results For MAP,HR,SpO2 and RR,there was a significant difference between the two groups ( P 〈0.05). Adverse reactions such as salivation, nausea, vomiting in Group A were obviously lower than those in Group B; During BP,HR and SpO2 monitoring, BP was markedly high in Group B;and 18 cases were in the high level above 20% normal. But BP in Group A was differently decreased, and 12 cases were lower 20% level than normal. There was a significant difference ( P 〈 0.05) .SpO2 in Group A were also decreased,and much lower than that in Group B( P 〈 0.01) .Conclusion Propofol and fentanyl in combination for gastroscopic examination may have less adverse reactions, keep circulation stable during operation, and reduce operation time and complications.
