摘要
目的介绍改良直线缝合法修复双侧唇裂的方法和体会。方法在VeauⅢ术式的基础上,于双侧唇裂的两侧唇部设计A、B皮肤黏膜瓣,衬垫于前唇部的内侧,以增加创面的接触面积并加厚前唇部;在前唇部上方内侧设计C黏膜瓣加深唇龈沟,并将两侧唇肌肉分离固定于鼻前嵴、鼻翼基底及前唇,以达到唇肌的功能性修复。结果本组12例患者,术后上唇及唇红较丰满,动态畸形不明显,效果满意。结论采用改良直线缝合法修复双侧唇裂,能增加切口抗张力和改善术后上唇形态,减轻术后动态畸形及上唇塌陷。同时,此法还能保留较多的上唇组织,有利于唇裂Ⅱ期畸形的修复。
Objective To introduce an improved Veau Ⅲ procedure to treat bilateral cleft tip. Methods We designed a three - flap procedure on the basis of Veau Ⅲ, within flap A and flap B from lateral lips on the bilateral cleft lip were placed into the wall of the anterior lip to enlarge the contact surface area and thicken the anterior lip; then flap C from upper wall of the anterior lip was used to deepen the labiogingival groove; the bilateral cleft lip muscles were separated and fixed it at the anterior nasal spine, nasal wing base and anterior tip for functional repair of lip muscles. Results In all 12 cases we operated on, desired results were obtained： the postoperational upper lips were thickened together with good shape and vermilion color, and there was no obvious dynamic abnormality. Conclusion This improved method can strengthen incisional tissue tension- resistant intensity, improve postoperational anterior lip flatness and reduce dynamic abnormality, and keep more upper lip tissues for further reparation at state Ⅱ .
