摘要
目的:探讨血清可溶性转铁蛋白受体(sTfR)水平对慢性炎症患者并发缺铁性贫血与慢性炎症患者并发非缺铁性贫血的鉴别诊断价值。方法:采用散射免疫比浊法定量测定非贫血对照组,单一原因造成的缺铁性贫血(IDA)、慢性炎症并发缺铁性贫血患者、慢性炎症并发非缺铁性贫血患者sTfR浓度。采用t检验和ROC曲线评价sTfR的诊断价值。结果:非贫血对照组与单一原因造成的IDA sTfR浓度有显著性差异(P<0.05),AUC=0.965,有较高的诊断准确度。慢性炎症并发缺铁性贫血患者与慢性炎症并发非缺铁性贫血患者sTfR浓度有显著性差异(P<0.05);AUC=0.834,有一定的准确度。结论:sTfR浓度对单一原因造成的IDA和慢性炎症并发缺铁性贫血患者有较高的诊断准确性;并且根据不同的临床资料选用不同的阈值用于诊断。
Objective To evaluate the differential diagnostic importance of serum soluble transferrin receptor (sTfR) for iron deficiency anemia (IDA) of chronic inflammation from non iron deficiency anemia of chronic inflammation. Methods By nephelometry, we detected the sTfR concentrations in: ① nonanemic controls, n =55 ② patients with simple IDA, n = 16 ③patients with IDA of chronic inflammation, n = 19 ④ patients with non iron deficiency anemia of chronic inflammation (n = 20) respectively. We used t - test and ROC curve to evaluate the diagnostic value of sTfR. Results The sTfR concentrations were significantly different between those in the nonanemic controls and those in simple IDA group (P 〈 0.05 ). AUC = 0. 965, the results had considerable diagnostic accuracy. There were also significant differences between the sTfR concentrations of IDA of chronic inflammation and non iron deficiency anemia of chronic inflammation (P 〈 0.05 ), AUC = 0. 834, which had substantial accuracy. Conclusion Determination of sTfR concentration was of considerable accuracy for the diagnosis of simple IDA and IDA of chronic inflammation. A higher threshold value should be applied in patients with chronic inflammations.
