摘要
为了探索常温下(22℃)保存离体肝脏的可能性,本实验以HTK液为基础,氟碳乳剂为氧载体,脂肪乳剂为能量,常温下充氧持续灌注离体猪肝门静脉。定时取灌注液测量肝生化指标,观察肝细胞线粒体大小及密度。发现含氧载体灌注液中氨浓度下降,尿素浓度升高,线粒体损伤较轻;加入脂肪乳剂则能维持葡萄糖的浓度。各项指标与对照组比较差异有显著意义。说明利用氧载体(氟碳乳剂)为离体肝提供足够的氧,同时给予足够的能量(脂肪乳剂)时,离体肝脏就能在较高温度下较长时间(40小时)地维持它的主要解毒功能——摄取氨合成尿素,同时亦可维持肝细胞线粒体的稳定性。可见常温下保存离体肝脏是可能的。
Toprovethatperfusedlivercanbepreservedatroomtemperature(22℃),Wemadetheexperiment,theinwhichHTKwasbasicsolution,perfluocarbonactedasoxygencarrierandlipidactedasenergysubstrateinthehomoiothermycondition,pigliverorganswereperfusedextracorporealythroughtheV.portalwithperfusionsolution.Infixedperiodoftimetheperfusionsolutionfromtheliv-erwastakenandanalysedtodetermineforliverbiochemicalfunctionandobservetheconcentrationandsizeofhepatocelularmitochondria.Inoxygencarrierperfusionsolutiontheammoniaconcentrationwaslow,ureaconcentrationwashigh,damagetomitochondriawasminimumandbyadditionoflipidemul-siontheconcentrationofglucosecanbemaintained.Theexperimentalandcontroldatawereobviouslysignificant.Theresultdemonstratedthatoxygencarrier(perfluocarbon)asoxygensuppliercanprovidesenoughoxygentolivercel,atthesametime,lipidmulsion(intralipid)providesenergysoastheper-fusedlivercanbepreservedathightemperature,i.e.22℃atrelativelongtime(40hours)andstilhasthefunctionofremovingtoxicsubstancesuchasammoniaandconvertingittourea,meanwhilemaintainthenormalstructureofmitochondria.Thepreservationofperfusedliverathomoiothermyispossible.
