羟乙基淀粉急性等容性血液稀释对病人凝血状态的影响 被引量:8

Effects of acute normovolemic hemodilution with hydroxyethyl starch on blood coagulation

摘要 目的探讨羟乙基淀粉急性等容性中等程度血液稀释后病人凝血状态的变化。方法10例择期手术病人用羟乙基淀粉130/0.4氯化钠注射液进行血液稀释。分别于血液稀释前及稀释后15min测定血常规,包括血红蛋白(Hb)、红细胞压积(Hct)、血小板计数(PLT);测定传统凝血实验指标。包括凝血酶原时间(PT)、部分激活凝血活酶时间(AFTT)、纤维蛋白原(Fib),同时用血栓弹性描记仪测定参数反应时间(R)、凝血时间(K)、α角(ANG)、最大振幅(MA)、A30、A60、CL3、CL60、LY30和LY60。结果血液稀释后Hb及Hct均减少,PT延长,Fib减少(P<0.01),而APTT及血栓弹性描记仪测定指标的变化均无统计学意义(P>0.05)。结论羟乙基淀粉130/0.4氯化钠注射液进行急性等容性中等程度血液稀释后凝血功能没有明显变化。 Objective To investigate the changes in blood coagulation after acute normovolemic hemodilution (ANH) with hydroxyethyl starch (HES) .Methods Ten ASA Ⅰ or Ⅱ patients undergoing elective operation were studied. ANH was performed by removing 10 ml· kg^- 1 of blood which was replaced by simultaneous infusion of equal volume of HES (130/0.4 Voluven) at the same speed. Blood routine (Hb, Hct, platelets), standard coagulation tests (prethrombin time, activated partial thromboplastin time, fibrinogen) and TEG (R, K, alpha angle, MA, A30, A60) were performed before and 15 rain after ANH. Results Hb and Hct were significantly decreased after hemedilution. Prothrombin time was significantly prolonged and plasma fibrinogen concentration was significantly reduced after ANH. APTT and TEG variables were not affected after hemedilution. Conclusion There is no significant change in blood coagulation after moderate ANH with HES (130/0.4 Voluven) .
