新生儿重度窒息与全身炎症反应综合征关系探讨 被引量:2

摘要 目的探讨全身炎症反应综合征(SIRS)与新生儿重度窒息的临床关系。方法对324例新生儿重度窒息按照SIRS诊断标准分SIRS组(156例)与非SIRS组(168例)比较两组脏器损害者发生率、病死率;比较S3组(符合SIRS诊断3项者)与S2组(符合SIRS诊断2项者)发生脏器损害程度及病死率。结果SIRS发生率为48.12%(156/324),在SIRS组与非SIRS组间脏器损害的发生率分别为78.21%,45.24%,2个以下脏器损害的发生率分别为62.82%,39.29%,多脏器损害的发生率分别为15.38%,5.95%,病死率分别为7.69%,1.79%,差异均有非常显著性(P<0.01);在S2组与S3组,轻度-中度脏器损害发生率分别为62.12%,50.00%,两者差异无显著性(P>0.05);重度脏器损害者发生率分别为17.42%,41.67%,病死率分别为4.55%,25.00%,差异均有非常显著性(P<0.01);SIRS组中几乎所有符合SIRS诊断的病例均有低体温。结论SIRS与新生儿重度窒息并发脏器损害有密切相关性;SIRS可以区别病情的严重程度及预测预后,应重视低体温。 Objective To explore the relationship between systemic inflammatory response syndrome (SIRS) and severe neonatal asphyxia. Methods 324 newborn infants with severe asphyxia were divided into SIRS group ( 156 cases) and non - SIRS group ( 168 cases) according to the 4 items of diagnostic criteria for SIRS and compared their occurrence rates mortality. Results The incidence of SIRS was 48.12% (156/324) ;The incidence of organ dysfunction was 78.21% Vs 45.24% (P 〈 0. 01 ) ;the incidence of organ dysfunction that was no more than two was 62.82% Vs 39.29% (P 〈0.01 ) ;the incidence of NODS was 15.38% Vs 5.95% (P 〈 0.01 ) ; the mortality was 7.69% Vs 1.79% ( P 〈 0.01 ) between SIRS group and non - SIRS group; The mild and middle damage was 62.12% Vs 50.00% (P 〉 0.05) ,the severe damage was 17.42% Vs meet two indexes of SIRS and S3 41.67% (P 〈 0.01 ) ,the mortality was 4.55% Vs 25.00% (P 〈 0.01 ) between S2 group and S3 group meet three indexes of SIRS. Almost all cases of SIRS group had low - temperature. Conclusion SIRS have close relationship with severe asphyxia complicating the organ dysfunction. The diagnostic criteria for SIRS used to identity the severity of the disease and also predict the outcome. We should pay more attention to the low - temperature.
