肾上腺交感神经离断术对利血平诱导大鼠肾上腺髓质增生的影响

摘要 目的:探讨大鼠肾上腺交感神经离断术在利血平诱导的大鼠肾上腺髓质增生中的作用及肾上腺交感神经与肾上腺髓质增生的关系。方法:雄性Wistar大鼠60只随机分为A、B、C、D 4组各15只,A组为单侧肾上腺交感神经离断组,B组为假手术组,C组为肾上腺髓质增生组,D组为正常对照组。C组用利血平诱导肾上腺髓质增生大鼠模型,A、B组手术7 d后同C组给予相同剂量利血平42 d。分别于停药后第14,28,56天时测大鼠24 h尿3-甲基-4-羟基苦仁酸(VMA)含量及血浆儿茶酚胺(CA)水平,处死大鼠,取肾上腺做病理切片,观察肾上腺髓质增生情况并测肾上腺髓质百分数。结果:在各观察阶段,A、B、C组大鼠24 h尿VMA和血浆CA含量明显高于正常对照组(P<0.05),A组手术侧肾上腺百分数明显低于未手术侧(P<0.05),而与正常对照组无明显差异(P>0.05)。结论:大鼠肾上腺交感神经离断术可抑制利血平诱导大鼠肾上腺髓质增生的作用,利血平通过肾上腺交感神经的介导作用于大鼠肾上腺髓质引起增生,肾上腺交感神经离断术可能对肾上腺髓质增生有治疗作用。 Objective: To study the effect of unilateral adrenal denervation in rat adrenal medulla hyperplasia (AMH) induced by reserpine and the relationship between adrenal sympathetic nerve and adrenal medulla hyperplasia. Methods: Sixty male Wistar rats were randomly divided into four groups, Group A were unilateral adrenal denervated, Group B were sham operated, Group C were AMH group and Group D were normal control. Seven days after surgery. Group A and B were subcutaneously injected with reserpine with the same dose as Group C for 42 days. 3-methoxy-4-hydroxyvanillymandeic acid(VMA) content in urine and catecholamine(CA) level in plasma were determined at the 14th, 28th and 56th day after injection. Pathologic studies of adrenal were done. Results: At different time points, the values of VMA and CA in groups A, B and C were higher than those in Group D (P〈0. 05). Medulla percentages of denervated glands were lower than those of glands with intact innervation in Group A (P〈0.05), but similar to those in normal control group (P〉0.05). Conclusion: Rat adrenal denervation prevents adrenal medulla proliferation induced by reserpine, neurogenic control of cell proliferation plays an important role in the pathogenesis of AMH by reserpine, and adrenal denervation may be effective in cure of AMH.
