血红蛋白与血气分析对支气管动脉造影与灌注化疗肺癌患者的预警作用

摘要 目的探讨血红蛋白(Hb)和血气分析对在支气管动脉造影与灌注化疗术的预警作用。方法对45例肺癌患者在术前后2h的Hb与血气分析进行对比。结果术后患者血氧分压(PaO2)下降,二氧化碳分压(PaCO2)升高,与术前分别比较差异有显著性(P<0.05);术后患者出现胃肠道反应、呼吸衰竭、心律失常、休克血压、便秘的发生率分别为13.33%、4.44%、4.44%、2.22%、2.22%;实施支气管动脉造影与灌注化疗相对安全。结论行支气管动脉造影与灌注化疗治疗肺癌时,密切结合临床表现和病人体质特点,进行Hb与血气分析检查,对预测预后和防止并发症有重要意义。 Objective To estimate the predictive values of hemoglobin (abbreviated as Hb) and blood gas analysis in predicting the prognosis in bronchography and perfusion chemotherapy. Methods 45 patients with lung cancer were involved in this study, 2 hours before and after bronchography and perfusion chemotherapy, levels of Hb and blood gas analysis were compared. Results After operation, decreased PaO2 and elevated PaCO2 were found, in comparison to those before operation, the two indexes differed significantly ( P 〈0.05). As to the postoperative side effect, the occurrences of gastrointestinal reaction, respiratory failure, arrhythmia, shock blood pressure and constipation were 13.33%, 4.44%, 4.44%, 2.22% and 2.22%, respectively. Bronchography and perfusion chemotherapy were relatively safe. Conclusion While performing bronchography and perfusion chemotherapy, Hb and blood gas analysis examination in combination with clinical symptoms and individual health state are of great values in predicting prognosis and preventing complication.
