摘要
目的观察眼前节内眼手术中由前房穿刺导致的前房水污染情况,并比较术前用庆大霉素注射液、5g·L-1国产聚维酮碘和50g·L-1进口聚维酮碘冲洗结膜囊降低前房污染的效果。方法300例(300眼)行眼前节手术的患者,随机分为3组。患者术前1d用3g·L-1氧氟沙星滴眼液滴患眼6次。术前约5min依照分组分别用8g·L-1庆大霉素注射液、5g·L-1国产聚维酮碘及50g·L-1进口聚维酮碘冲洗结膜囊。术中用前房穿刺刀做好透明角膜辅助切口,没有用灌注液之前,采集前房水作细菌培养。结果用药后庆大霉素组、5g·L-1聚维酮碘组及50g·L-1聚维酮碘组前房水细菌培养阳性率分别为2.7%、6·5%和5.3%,平均为4·9%,3组间差别没有统计学意义(P>0.05)。结论前房穿刺可以将附着在眼表的细菌带入前房。术前用庆大霉素注射液、5g·L-1国产聚维酮碘和50g·L-1进口聚维酮碘冲洗结膜囊清除眼表细菌的效果相似。
Objective To observe the contamination of anterior chamber aqueous induced by anterior chamber puncture in anterior segment intraocular surgery,and compare the effects of Gentamycin,5 g · L^-1 domestic-made Povidoue-iodine and 50 g · L^-1 imported Povidone-iodine on reducing this contamination by irrigating conjunctival sac before surgery. Methods Three hundreds consecutive patients (300 eyes) were divided into three groups: Gentsmycin group, 5 g · L^-1 Povidone-iodine group and 50 g · L^-1 Povidone-iodine group. All patients were received topical 3 g · L^-1 Ofloxacin 6 times in one day before surgery. About 5 minutes before surgery, conjunctival sac was irrigated with 8g · L^-1 Gentamycin, 5 g · L^-1 Povidoue-iodine and 50 g · L^-1 Povidone-iodine, respectively. The clear corneal incision was made by paracentesis knife in the surgery. Before irrigated, the anterior chamber aqueous was collected to cttlture. Results The incidence of anterior chamber aqueous contaminations in Gentamycin group ,5 g · L^-1 Povidone-iodine group and 50 g · L^-1 Povidone-iodine group were 2. 7% ,6.5% and 5.3% respectively. The average incidence was 4.9% and no statistically difference was found among the three groups(P〉0.05). Conclusion The anterior chamber puncture can take the bacteria adjacent to the ocular surface into anterior chamber. Irrigating with Gentamycin,5g · L^-1 Povidone-iodine and 50g · L^-1 Povidoneio-dine before operation have similar effectiveness in reducing the rate of contamination.
