摘要
目的:分析白大衣高血压患者24 h动态血压的特征,为其防治提供依据。方法:测量白大衣高血压80例、高血压病Ⅰ级80例和正常血压60例的诊室血压和动态血压,对其均值、负荷值、昼夜节律和变异性进行组间比较。结果:白大衣高血压组的诊室血压均值高于高血压病Ⅰ级组诊室血压均值(P<0.01),白大衣高血压组白昼及夜间收缩压和舒张压负荷值,昼夜血压类型比率均介于正常血压组和高血压病组之间(P<0.01,<0.05),白大衣高血压组动态血压各时段变异性明显高于高血压病组和正常血压组(P<0.01,<0.05),高血压病组亦高于正常血压组(P<0.01)。结论:白大衣高血压是血压异常状态,降低其变异性和负荷值,调整昼夜节律是防治白大衣高血压的目标。
Objective: To analysis the characteristic of 24 h ambulatory blood pressure (ABP, mmHg) of white coat hypertension (WCH) patients so that to provide theoretical basis for prevention and treatment of WCH. Methods: The clinical blood pressure (BP, mmHg) and ABP were measured in 80 WCH, 80 patients with essential hypertension (EH) grade Ⅰ and 60 normal BP controls, and the mean BP value, BP loading value, circadian rhythm and variability of ABP were analyzed in three groups.. Results: The mean clinic BP of WCH was markedly higher than that of EH grade Ⅰ group (P〈0.01). The nighttime or daytime blood pressure loads of SBP or DBP and the ratios of different blood pressure types of WCH group were between those of normal BP controls group and EH grade Ⅰ group (P〈 0.01, 〈0.05 ). ABP time-phase variability of WCH was markedly higher than that of EH grade Ⅰ group and normal BP controls (P〈0.01, 〈0. 05 ). ABP time-phase variability of EH grade Ⅰ group was markedly higher than that of normal BP controls (P〈0. 01). Conclusion: Treatment must be taken to reduce the variability and loading values and to regulate circadian rhythm of ABP in WCH since blood pressure of WCH is abnormal.
