TTT治疗老年性黄斑变性CNV和高度近视CNV的视功能比较 被引量:1

Visual function before and after transpupillary thermal therapy of subfoveal choroidal neovascularization in age-related macular degeneration and high myopia

摘要 目的应用经瞳孔温热疗法(TTT)治疗老年性黄斑变性(AMD)合并脉络膜新生血管(CNV)和高度近视合并CNV,比较治疗后的黄斑平均视敏度(MS)、平均缺损(MD)、视力等变化。方法收集湿性AMD28例32眼和高度近视合并CNV16例20眼。所有患者治疗前后均行眼底、视力、FFA、OCT等检查确诊。应用IRISmedicalOculightSLx激光仪进行治疗。用SPSS10统计分析TTT治疗后3m、6m的视力及中心视野M2程序中MS、MD值。结果治疗1m时,两组治疗有效者分别为25/32眼;16/20眼。治疗3个月时,两组治疗有效者分别为28/32眼;16/20眼。两组间差异无意义(P=0.58;0.36)。治疗6m时,两组治疗有效者分别为28/32眼;12/20眼,两组间差异有显著意义(P=0.02)。AMD合并CNV者TTT治疗前MS值13.70±5.03dB,治疗后3m、6m的MS值分别为16.30±6.2dB,16.90±5.93dB(P=0.088;0.037)。TTT治疗前MD值13.58±4.88dB,TTT治疗后3m、6m的MD值分别为11.27±5.32dB,11.76±4.84dB。(P=0.084;0.172)。高度近视合并CNV者TTT治疗前MS值14.70±4.85dB,TTT治疗后3m、6m的MS值分别为16.62±5.09dB,14.90±4.21dB(P=0.206;0.897)。TTT治疗前MD值14.46±5.21dB,TTT治疗后3m、6m的MD值分别为13.29±4.59dB,15.34±4.38dB(P=0.436;0.562)。结论TTT对CNV有治疗作用,对周围组织存在相对较小的损伤,能保持和提高视功能。6m时疗效AMD组好于高度近视组。 Objective To evaluate the therapeutic effect of transpupillary thermal therapy (TIT) for subfoveal choroidal neovascularization (CNV) caused by age-related macular degeneration (AMD) and high myopia through the visual function including macular medial sensitivity (MS), medial default (MD),visual acuity and macular thickness. Methods twenty-eight cases (32 eyes) with exudative AMD and sixteen cases (20 eyes) with high myopia were treated with TTT(IRIS medical Oculight SLx laser apparatus). Visual acuity, FFA, OCT were evaluated before and after treatment. The follow-up was 3-6 months. Visual acuity and MS and MD in central visual field M2 progress were analysed by SPSS10 progress during the follow-up. Results At one months after treatment, TTT has therapeutic effect in 25/32 and 16/20 cases of AMD group and high myopia group repectively. At three months after treatment, TTThas therapeutic effect in 28/32 and 12/20 cases of each group. The difference between two groups was no significant (P=0.58; 0.36). But at six months after treatment, TTT has therapeutic effect in 28/32 and 16/20 cases of each group. The difference of visual acuity changes between two groups was significant (P=0.02). Six months after treatment, MS of AMD group increased. The difference was significant (P=0.037). The difference of MS and MD in high myopia group were no significant during the follow-up. Conclusion TTT has curative effect on CNV, and it can help to keep or improve the visual function. The damage of TTT to surrounding tissue is correspondingly less than other laser therapy. At six month, the curative effect of TTT to CNV of AMD is better than to CNV of high myopia .
