摘要 目的探讨急性脑出血患者(ACH)血浆细胞因子、粘附分子的动态变化规律、与脑水肿严重程度的关系。方法测定40例健康对照者及60例ACH患者发病24小时内、72小时、10天及35例ACH患者15天时血浆白介素-1β(IL-1β)、肿瘤坏死因子α(TNFα)、细胞间粘附分子-1(ICAM-1)含量,并对脑出血患者在相应时间检查头颅CT,计算脑水肿大小,对结果进行分析。结果(1)ACH患者不同时期血浆IL-1β、TNFα、TCAM-1含量均明显高于正常对照组(P<0.01);(2)ACH患者发病24小时内、72小时、10天时血浆IL-1β、TNFα、ICAM-1含量变化均无显著差异(P>0.05),直到发病15天时血浆IL-1β、TNFαI、CAM-1含量明显下降(P<0.01);(3)ACH发生后不同时期血浆IL-1β、TNFαI、CAM-1含量与脑水肿程度均呈正相关。结论细胞因子、粘附分子参与了脑出血的脑水肿形成,早期应用细胞因子拮抗剂、积极预防和控制感染对于控制脑水肿、改善预后至关重要。 Objective To investigate the content of inflammatory cytokines in the patients with acute cerebral hemorrhage (ACH) and its clinical significance. Methods Plasma interleukin-1β(IL- 1β) ,turner necrosis factor- alpha (TNF α) and intercellular adhesion molecule - 1 (sICAM - 1) were measured in 40 healthy persons and 60 patients after the onset of ACH 24h, 72h, 10d and 15d. The size of brain edema on computed tomographic (CT) scan in ACH patients was determined at the corresponding time. Results (1) The plasma contents of IL-1β, TNF α, ICAM - 1 in ACH patients were all significantly higher than those in healthy controls ( P 〈 0.01). (2) There was no significant difference for plasma contents of IL- 1β, TNF α, ICAM- 1 in ACH patients after onset at 24h, 72h and 10d(P〉 0.05). However, plasma eontents of IL- 1β, TNFa, ICAM - 1 in ACH patients after onset on 15d were apparently lower than those at 24h, 72h and 10el ( P 〈 0.01). (3) There was a positive correlation between the size of brain edema and the plasma contents of IL- 1β, TNFα, ICAM- 1 in ACH patients at the different period. Conclusions The plasma contents of the inflammatory cytokines can reflect the severity of the brain edema in ACH patients. To use the antagonist of inflammatory cytokines in early stage and to prevent and control the inflammation are very important for reducing brain edema and improving the prognosis.
