超大腓肠神经营养血管皮瓣修复足踝部皮缺损 被引量:12

摘要 目的探讨超大腓肠神经营养血管逆行岛状皮瓣的临床应用。方法对13例足踝包括前足底部的软组织缺损的患者,采用超大腓肠神经营养血管逆行岛状皮瓣移位修复。皮瓣面积最大32×12cm,最小16×8cm。结果13例中得到远期随访10例,术后皮瓣外观色泽均正常,足外侧缘皮肤感觉早期减退,术后3~6个月全部或部分恢复。而移位的皮瓣厚薄适宜,质地及功能良好,6个月后均恢复了保护性感觉。结论超大腓肠神经营养血管逆行岛状皮瓣切取简便,血供可靠,不牺牲主要动脉,由于旋转点的演变,轴点下移,使踝关节包括前足的大面积皮肤缺损均能获得有效的覆盖,是修复足踝部皮肤缺损的理想方法。 Objective To approach the clinical application of supramaximal reversed sural neurocutaneous island flap.Methods 13 cases of soft tissue defect around the ankle of foot were repaired with the supramaximal sural neurocutaneous island flaps.The and anterior part largest flap size was 32cm×12cm,while the smallest was 16cm×8cm.Results The follow-up of the 10 flaps showed that all flaps were satisfactory. The flaps had fine texture and well distributed on thickness and colour.six month later,all the flaps regained protective sensation.The sensation of lateral border of foot was decreased in the earlier period, and it would regained after threre of six month. Conclusion Since the blood supply of the supramaximal reversed sural neurocutaneous island flap is reliable,and the flap is easy and quick to get without sacrificing the major arteries.With the changing of swivel,even a large area soft tissue defect around the ankle and anterior part of sole can be repaired satisfactory.It is an ideal method to use this flap to repair the soft tissue defect around the ankle.
