摘要
目的:观察异丙酚静脉麻醉在无痛胃镜检查中应用的效果、安全性及不良反应。方法:麻醉组380例给予异丙酚静脉注射麻醉后行胃镜检查。对照组180例行常规胃镜检查。观察两组患者检查前、中、后血压、心率、血氧饱和度的变化及患者的反应,麻醉组清醒时间。结果:麻醉组患者对检查感受的不适程度明显低于对照组(P＜0．05);清醒时间1～5 min,平均(2．7±1．3)min;检查过程中有一过性血压下降、心率减慢,但停药后迅速恢复。而对照组检查过程中有不同程度的血压升高、心率加快。两组检查前后血压、心率、血氧饱和度均无统计学差异。麻醉组94．5%的患者愿意接受再次胃镜检查,对照组仅22．8%的患者愿意接受必要的复查。结论:异丙酚静脉麻醉用于无痛胃镜检查是一种安全有效的方法。
Objective： To investigate the safety and efficacy as well as the adverse reactions of sedation and anesthesia with intraveneous propofol under no-pain gastroscopy. Method： Gastroscopy was preformed in 380 patients of the anesthesia group with intraveneous propofol and in 180 patients of the controlled group with traditional methods. The changes of blood pressure, heart rate and blood oxygen saturation were recorded and analyzed. Patients＇satisfaction and reaction and waking time in the anesthesia group were compared with those of the controlled group. Result： The degree of discomfort in the anesthesia group was remarkably lower as compared with that of the controlled group（ P 〈 0.05 ）. The recovery time of the anesthesia group was 1-5 （ 2.7 ± 1.3 ） min. The blood pressure and heart rate were decreased in the anesthesia group during the gastroscopic procedure and recovered after gastroscopy. But the blood pressure and heart rate increased in the controlled group. The blood pressure, heart rate and blood oxygen saturation before the procedure and after the procedure in both groups had no significant difference in statistics. 94.5% patients of the anesthesia group were willing to accept no-pain gastroscopy again, but only 22.8% patients of the controlled group did it again. Conclusion： Propofol in the operation of gastroscopy is safe and effective.
