摘要
目的探讨91例年龄≤35岁妇女宫颈癌的不良预后因素。方法对91例年龄≤35岁宫颈癌患者的临床资料进行回顾性分析。91例中,24~30岁35例,30~35岁56例。16~20岁结婚49例。鳞癌76例,腺癌6例,其他病理类型9例。Ⅰ期49例,Ⅱ期35例,Ⅲ期6例,Ⅳ期1例。手术治疗60例,根治性放疗31例。结果91例治疗结束时均完全缓解,半年内出现复发、转移者为54例(59.3%),1年内77例(84.6%)。复发、转移部位以原发灶紧邻区域为主61例(67.0%),沿淋巴结转移8例(8.8%),远处转移22例(24.2%)。结论早婚、早育、多产,症状持续时间短、肿瘤体积大为不良预后因素,病理类型不是年轻宫颈癌不良预后因素的首要指标。临床诊疗过程中应重点关注盆腔病变及相邻区域的处理,探讨减少远处转移的有效化疗方法。
Objective To explore the poor prognosis factors of young women patients with cervical cancer whose age being ≤35 years old. Methods To retrospectively analyze the 91 recurrence of young women cervical cancer of recurrence and metastasis patients who being or less than 35 years old treated in Hunan Provincial Tumor Hospital from Oct 1978 to Jan 2005, among which 35 patients being 24~30 years old, 56 patients being 30~35 years old, and 49 patients married at 16~20 years old,76 cases of squamous carcinoma, 6 cases of adenocarcinoma, 9 cases of other histologic type. 49 cases of Ⅰ stage, 35 cases of Ⅱ stage, 6 cases of Ⅲ stage, 1 case of Ⅳ stage, 60 cases received operation, 31 cases received radical radiotherapy. Results All the symptoms of 91 cases were released after being treated. The recurrence or metastasis rate was 59.3 % in a half year and 84.6 % in one year, respectively. The main (lesion) malfunction positions were the tissue (organ) adjacent to the primary origin sites (67.0%), while the lymph node metastasis rate 8.8%, and remote (other organs) metastasis rate was 24.2%. Conclusion Early marrying, early bearing and too much delivery, and short duration of symptom and large tumor are the poor prognosis factors for cervical cancer. Histologic type is not the chief index for judging the poor prognosis factors of young patients with cervical cancer. During clinical diagnosis and therapy, attention should be paid to the treatment on malfunction of pelvis and adjacent organ and the effective chemotherapy on decreasing other organs metastasis should be adapted.
