摘要
目的探讨丙泊酚在内镜检查操作中的镇静作用及护理支持。方法随机选择接受胃肠镜检查者1 342例,其中自愿接受静脉麻醉的637例为实验组,选用手背静脉推注丙泊酚2mg/kg;接受普通胃肠镜检查者705例为对照组。记录患者接受检查前后不同时间的血压、心率、血氧饱和度,观察操作时间、清醒时间、不良反应及患者的主观意见反应。结果实验组不良反应发生率明显降低(P<0.01)。检查中血压、心率较检查前有不同程度下降,患者满意度和愿意再接受检查的比例明显高于对照组(P<0.01)。结论应用丙泊酚静脉麻醉可显著提高内镜操作时患者的耐受性,具有较好的安全性,且护理支持在心电监护、供氧、建立静脉通路、镇静观察等至关重要。
Objective To explore the sedative effect and nursing of Propofol used in gastrointestinal endosocopic manipulation. Methods 1342 patients underwent gastroscopy or colonoscopy were divided randomly into two groups： experimental group （n = 637） and control group （n = 705） . In experimental group, propofol （2 mg/kg） was intravenously injected for the patients before gastroscopy or colonoscopy. In control group, the patients underwent conventional endoscopy. Blood pressure （Bp）, heart rate （HR） and oxygen saturation （SaO2） of patients in two groups were monitored before and after endoscopy. During the examination, the patients＇ examination time, consciousness recovery time, side-effects and their feelings were recorded. Results The occurrence rate of side-effects of experimental group decreased significantly （P 〈 0.01 ）, and their BP and HR decreased during the examination. Compared with control group, the satisfaction rate and re-acceptance rate of experimental group increased significantly （P 〈 0.01 ）. Conclusions Propofol is safe and can ameliorate the patient＇s tolerance during andoscopy . The nursing such as electrocardiac monitor, oxygen support, establishment of venous injection, observation of sedation are important.
