摘要
目的:为肾脏疾病及腰疝的手术提供解剖学依据。方法:从腹后壁外面观察腰上三角的形态及构成,由其内面观察经过该三角前方的肋下神经、髂腹下神经和髂腹股沟神经的组合形式及走行,并分别测量其与第12肋下缘的距离。结果:腰上三角的彤态可为四边形(55%)、三角形(35%)和不规则形(10%),该三角的上界可完全由第11肋(5%)构成;肋下神经、髂腹下神经和髂腹股沟神在腰上三角前方有4种不同的组合形式,三神经与第12肋下缘的平均距离分别为(16.92±6.32)mm、(59.49±14.97)mm、(112.49±17.15)mm。结论:熟悉腰上三角前方肋下神经、髂腹下神经和髂腹股沟神经的走行及与第12肋的位置关系,对于避免手术中损伤这3条神经具有重要的实际意义。
Objective: To provide anatomical basis for operative procedures of renal diseases and lumbar hernia. Methods: The shape and formation of superior lumbar triangle were observed from the external surface of posterior abdominal wall. The grouping patterns and courses of subcostal nerve, iliohypogastric nerve and ilioinguinal nerve passing the front of superior lumbar triangle were also observed. The distances between inferior margin of 12th rib and the 3 above-mentioned nerves were all measured. Results: The superior lumbar triangle was quadrilateral(55%), triangular(35%) or irregular(5 % ). If the 12th rib was too short, the upper border of the triangle would be the llth rib(5%). Four grouping patterns of the 3 nerves were found in front of the triangle, and the average distance between inferior margin of 12th rib and the 3 nerves was (16.92±6.32)mm, (59.49±14.97) mm and ( 112.49± 17. 15) mm, respectively. Conclusion: An intimate knowledge of the courses and positional re lations to the 12th rib about subcostal nerve, iliohypogastric nerve and ilioinguinal nerve play very important roles in avoiding injury of the 3 nerves during the operation on kidney or lumbar hernia.
