HA型树脂灌流器治疗重度有机磷中毒的临床研究

摘要 目的研究HA型树脂灌流器治疗重度急性有机磷农药中毒的效果。方法将90例急性重度有机磷农药中毒者根据血液灌流(HP)分为:树脂HP组(治疗组)、炭肾HP组(对照组Ⅰ)、非HP组(对照组Ⅱ),比较治疗组和对照组在昏迷至清醒时间、阿托品化时间、应用阿托品总量、胆碱酯酶活力恢复时间、病死率、住院时间等的差异,比较树脂HP组和炭肾HP组的不良反应。结果树脂HP组和非HP组比较,患者昏迷至清醒时间、阿托品化时间、血清胆碱酯酶活力恢复时间、住院时间明显缩短(P<0.01或0.05),阿托品总量明显降低(P<0.01)、病死率降低(P<0.05);树脂HP组和炭肾HP组疗效、病死率比较差异无显著性(P>0.05),但树脂HP组血小板减少、致热反应的发生率低于炭肾HP组(P均<0.05)。结论应用HA型树脂灌流器治疗重度有机磷中毒疗效良好,且不良反应少。 Objective To summarize the clinical effects of HA - type resin hemoperfusion (HP) apparatus for treatment acute severe organophosphorus pesticide poisoning. Methods 90 patients with acute severe organophosphorus pesticide posoning enrolled in this study were allocated to three groups: resin HP group (therapeutic group), activated charcoal liP group (control group I) and non - HP group (control group 2). In these three groups, coma to waking time, atropinization time, total close of atropine, cholinesterase activity recovery time, mortality and in - hospital time were all recorded and compared among these three groups. Compared of adverse reaction between therapeutic group and control group i. Results In comparison to non- HP group, the patients in resin HP group needed shorter time for coma to waking, for atropinization, for serum cholinesterase activation recovery and for in -hospital ( P 〈0.01 or 0.05), lower total dose of atropine ( P 〈0.01), lower mortality ( P 〈0.05). Compared resin group to activated charcoal liP group, there were no statistical significant differences in curative effects and morbility ( P 〉0.05), but more thrombocytopenia and lower pyrogenic reaction rate in resin HP group were also found ( P 〈0.05). Conclusion HAtype resin hemoperfusion apparatus in treatment acute organophosphate insecticides poisoning is effective and has less side effect.
