摘要
目的:探讨survivin、MMP-9蛋白的表达与癌间质微血管密度(MVD)的值的关系及其对胃癌预后的影响。方法:采用免疫组化S—P法,检测128例胃癌手术标本中survivin、MMP-9、CD34的表达。结果:128例胃癌组织中survivin、MMP-9阳性表达率和MVD分别为60.9%、64.1%、28.68±10.25,三者均与胃癌浸润深度、淋巴结转移、TNM分期密切相关(P〈0.05);survivin阳性表达和MVD与肿瘤组织分化程度和远处转移有关(P〈0.05);三者在胃癌中表达呈正相关(P〈0.05);survivin、MMP-9共同表达者组发生深度浸润(T3-4)与淋巴结转移的比率和MVD明显高于其中之一或二者皆阴性表达者(P〈0.05);survivin、MMP-9蛋白阳性表达者术后生存率明显低于阴性者(P〈0.05)。结论:sunrivin、MMP-9与血管形成和胃癌生长、浸润、转移及预后均有密切相关,联合检测survivin、MMP-9、CD34表达可作为判断胃癌生物学行为和预后的重要指标。
Objective:To investigate the expressions of survivin and matrix metalloproteinase - 9 (MMP - 9), and their correlation with microvessel density (MVD) and prognosis in human gastric carcinoma. Methods: Expressions of survivin, MMP- 9 and CD34 were detected with S -P immunohistochemically in 128 gastric carcinoma patients. Results: The positive rates of survivin and MMP - 9 expressions in the studied patients were 60. 9% and 64.1% respectively. The mean MVD was 28.68 ± 10.25. The above parameters were closely related with tumor invasion depth, lymph node metastasis and TNM stages ( P 〈 0.05). Also survivin expression and MVD were associated with tumor differentiation and distant metastasis ( P 〈 0.05). Patients with both survivin and MMP - 9 expressions had deeper tumor invasion, more lymph node metastasis and higher MVD than the other ones ( P 〈 0.05). The survival rates after radical resection in patients with survivin or MMP - 9 expressions were significantly lower than those without ( P 〈 0.05). Conclusions: Expressions of survivin and MMP - 9 are associated with angiogenesis and tumor growth , invasion, metastasis and prognosis in gastric carcinoma. Therefore, combined detection of survivin, MMP - 9 and CD34 may be used as an important indicator for biological behavior and prognosis of gastric carcinoma.
