摘要
６６例功能失调性子宫出血（简称功血）患者阴道流血期间，血清泌乳素（ＰＲＬ）水平异常者为５６．１％，高ＰＲＬ血症及低ＰＲＬ血症者分别为４８．５％、７．６％，显著高于正常妇女组８．３％（Ｐ＜０．０１）。其中４５例确定了有无排卵。无排卵型功血占６８．９％，其异常ＰＲＬ血症率为７４．２％；有排卵型功血占３１．１％，其异常ＰＲＬ血症率为７１．４％，分别与正常对照组比较，差异均有显著性意义（Ｐ＜０．０１，＜０．０５）。８例功血患者于阴道流血期间和非阴道流血期间血清异常ＰＲＬ水平及其发生率均无显著性变化（Ｐ＞０．０５）。溴隐停治疗６例高ＰＲＬ血症功血患者，卵巢功能状态及功血均恢复正常，其中３例合并不孕症者正常妊娠、分娩，另３例血清ＰＲＬ恢复正常水平。提示，血清ＰＲＬ水平异常可能是中枢性调节垂体前叶ＰＲＬ分泌紊乱所致，进而影响卵泡发育成熟、排卵及黄体成熟各环节，是引起功血的病因之一。
Serun prolactin(PRL)levels were studied in 66 patients with functional bleeding in uteri(FBU)and 12 normal women served as controls. During uterine bleeding phase,the incidence of abnormal serum PRL levels(56.1% including hyperprolactinemia 48.5%and hypoprolactinemia 7.6%)in FBU group was significantly higher than that in controls(8.3%)(P< 0.01).Ovary function states in the patients with FBU were estimated,based on history,endometrial biopsy and BBT.In 45 cases of FBU patients,anovulatory type(68.9%)of these accompanied abnormalpolactinemia 74.2%,and ovulatory type(31.1%)accompanied abnormalpolactinemia 71.4%,which were significantly higher than that in controls (P<0.01 P<0.05),no significant between the two types of FBU patients(P> 0.05).During the two phases of bleeding and no bleeding in uteri, the incidence and level of abnormal serum PRL in 8 cases of FBU patients died not significantly change(P> 0.05).6 cases of FBU patients with hyperprolactemia which were treated by means of bromocriptin had been cured of secondary to overcoming their ovary function states. The results indicated that:the abnormal level of serum PRL may affect all stages of the development and adult of graafian follicle,ovulation corpus luteum and corpus albicans,is one of the causes resulting FBU;the abnormal level of serum PRL in FBU is concerned with composition and secretion in pituitary gland.
