摘要
目的:研究实验性2型糖尿病(T2DM)鼠在持续高血糖、高血脂状态下眼、肾组织结构的病理变化和特点。方法:①采用小剂量多次腹腔注射链脲佐菌素(STZ),同时给高热量高脂肪饲料喂养;②每天观察体重、尿量及眼变化,每2d测空腹血糖(FBG),成模后测空腹胰岛素(FINS)及血脂;③白内障出现后处死,取眼、肾在光镜和电镜下作组织结构观察;④肾小球细胞计数分析。结果:①T2DM组:成模后平均FBG(14.46±1.37)mmol/L,FINS(30.78±5.89)μU/ml,体重(494.26±72.73)g。对照组:FBG(5.17±1.36)mmol/L,FINS(19.59±4.89)μU/ml,体重(373.2±30.45)g;②T2DM组:在40~90d先后出现白内障。组织学检查,全眼球组织结构均有改变,虹膜、睫状体和视网膜毛细血管增生扩大,基膜增厚,睫状上皮退变。晶状体为退行性变。肾组织见肾小球多呈弥漫性硬化,肾小球细胞计数增多达80.9%,基质蓄积,基膜增厚,肾小球囊腔变窄或消失,系膜细胞细胞器增多,问质有胶原纤维素,足细胞次级突起变形或融合。结论:T2DM鼠在持续高血糖和高血脂状态下,眼球和肾组织出现糖尿病眼病(DO)及糖尿病肾病(DN)的改变,并且两者的病理改变基本是同步的。
Objective:To study the specialties in the chronic microangiopathy of the eyes and kidneys of experimental type 2 diabetes(T2DM) rats when they were in the conditions of hyperglycemic, hyperlipidemia and hypersulinemia. Methods: ①Hypo-dose streptozotocin(STZ) and diets with high fat and high cholesterin were used to induce type 2 diabetes in rats.②Rats in each group were frequently examined in weight, urine, eyes and fasting glycemic(FBG), fating plasma insulin(FINS), and Tofalcholesterol(TC), Trighyceride(TG) were also examined. ③These rats were killed in 40-90 days, and their eyes and kidneys were rapidly taken out and examined with light and electronic microscope. ④These glomerular cells were counted. Result:In the T2DM rats group, FBG mean value was (14.46±1.37)mmol/L, FINS(30.78±5.89) μU/ml, body weight value was (494.26±72.73)g. In control group, FBG mean value was (5.17±1.36) mmol/L, FINS(19. 59±4.89) μU/ml. The body weight mean value was (373.2±30.45)g. On the other hand in the T2DM rats group, their eyes had all developed cataract lenticular in 40-90 days. The capillarie of the iris, ciliary body and retina were increased and enlarged. The ciliary epithelium degenerated and there was thickening of the base membrane and degeneration of nucleus lentis. At a time, the kidney had developed microungiopath diffuse glomerularsclerois, glomerular hypertrophy glomerular cell proliferat. 80.9% had glomerular extracellular matrix accmulation and base-memdrane thickness. The capsular space was restricted or disappeared, glomerular mesengia cell organelle was increased; glomerular extracellular matrix had collagenous fibril and pedicle of podocyt, and some fused. Conclusion:Long lasted hyperglycemia and hyperlipidmia can lead to destruction of the eyes and kidneys, and histological structure change in diabetic rats may form the pathological basis of DO and DN. The damage of eyes and kidneys is synchronized.
