摘要
【目的】探讨胆红素对足月新生儿脐血Th1/Th2(T-helper type1/T-helper type2)功能状态的影响。【方法】收集并分离21例正常足月新生儿脐血单个核细胞(cord blood mononuclear cells,CBMC),分别置于含有不同浓度胆红素(0、60、90、120 mg/L)的牛血清白蛋白溶液中进行预先孵育,洗涤后经植物血凝素-P(phytohemagglutinin-P,PHA-P)刺激培养48 h,采用酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)检测培养上清液中白细胞介素-4(interleukin-4,IL-4)及干扰素-γ(interferonγ-,IFNγ-)水平。【结果】①经PHA-P诱生的IFN-γ水平120 mg/L组明显低于60 mg/L组、0mg/L组(Q=38.17、41.95,P<0.01),90 mg/L组亦明显低于60 mg/L组、0 mg/L组(Q=35.95、39.73,P<0.01),120 mg/L组与90 mg/L组比较,差异无显著性(Q=2.21,P>0.05),60 mg/L组低于0 mg/L组,差异有显著性(Q=3.78,P<0.05)。②各组培养上清液中IL-4产生水平差异无显著性(P>0.05)。【结论】生理性黄疸状态下胆红素水平≥60 mg/L时可进一步加重新生儿时期的Th1/Th2失衡状态。
[Objective] To study the effects of bilirubin on the functional state of Th1/Th2 in cord blood mononuclear cells (CBMC) of full-term neonates. [Methods] CBMC of 21 full-term neonates were in vitro preincubated with unconjugated bilirubin dissolved in bovine serum albumin solution at various levels (0,60,90,120 mg/L), washed, and lurther cultured with PHA-P for 48 hours. The levels of IFN-γ and IL-4 in the supernatants of CBMC were detected by ELISA. [Results] ①After the treatment with bilirubin of various concentration, the level of IFN-γ in 120 mg/l. was significantly lower than that in 60 mg/1. and 0 mg/L. (Q=38.17.41.95, P〈0.01). The IFN-γ level in 90 mg/1. was remarkably lower than that in 60 mg/L. and 0 mg/L. group (Q=35.95.39. 73,P〈0.01 ), and that in 60 mg/L, was lower than that in 0 mg/1.(Q= 3.78,P〈0.05). There was no difference of IFN-γ level between in 120 mg/L. and 90 mg/L. (Q= 2.21 ,P〉0.05). ③ No significant difference of IL-4 level in supernatants of CBMC was found among the four groups (P〉 0.05). [Conclusion] Further imbalance of Th1/Th2 could be caused by bilirubin in full-term neonate when the level of it in serum was higher than 60 mg/L.
