摘要
应用动物实验与临床研究相结合的方法,观察了低流量固定比例的氧化亚氮—氧麻醉期间FiO2、吸入气、肺泡气以及动脉血气变化。成年杂种犬吸22.5ml/kg·min-1~7.5ml/kg·min-1的N2O—O2,70%出现低氧,不能作为临床应用。临床手术病人吸入7.5ml/kg~7.5ml/kg和15ml/kg~7.5ml/kgN2O2-O2120min,理论上虽较安全,但仍有发生低氧之危险,麻醉时应有必要的监测措施。
To determine the safety and gas changes of lungs during low flow nitrous oxide-oxygen anesthesia, FiO2, gases of alveoli, inspired gases and blood gases were measured respectively. 15 mongrel dogs inhaled 7.5ml/kg~7.5ml/kg, 15ml/kg~7.5ml/kg and 22.5ml/kg~7.5ml/kg N2O-O2 per minute in a random order. 10 patients who belonged ASA risk Ⅰ, were devided into 2 groups (A and B). Group A were anesthetized with 7.5ml/kg~7.5ml/kg N2O-O2 per nminute, and group B done with 15ml/kg~7.5ml/kg N2O-O2 per minute. The dogs anesthetized with 22.5ml/kg~7.5ml/kg N2O-O2 had hypoxemia with PaO2 5.9 kpa. Ispired O2 concentrations (FiO2) were maintaned above 43% and 22% PaO2 were 23.7 and 13.9 kpa respectively in patients. But in group B,there were 2 patients with low FiO2 and PaO2. The results suggested that high concentration of N2O be avoided to patients, and pay attention to hypoxemia during anesthesia although low concentrations of low flow N2O-O2 are safe in theory.
