摘要
目的研究子宫内膜异位症(简称内异症)不育患者卵泡液中肿瘤坏死因子α浓度与体外受精胚胎移植(IVFET)结局的关系。方法收集我院2003年4月~2003年11月因子宫内膜异位症接受IVFET的20例患者(内异症组)与输卵管因素致不孕的20例患者(对照组)卵泡液,采用化学发光法测定肿瘤坏死因子α的浓度。结果内异症组促性腺激素用量(33.40±9.48)支,对照组(26.80±4.30)支,差异有显著性(P<0.05)。内异症组卵泡数(12.35±7.23),对照组(16.10±6.58)(P>0.05),差异无显著性。内异症组获卵数(10.8±6.87),受精率(0.57±0.22),胚胎数(5.95±4.32),均低于对照组(15.40±6.03),(0.72±0.16),(11.0±4.22),差异有显著性(P<0.05)。内异症组肿瘤坏死因子浓度(9.86±9.43)pg/ml,对照组(5.01±1.18)pg/ml,差异有显著性(P=0.034)。内异症组妊娠率26.3%(5/19),对照组42.1%(8/19),差异无显著性(P>0.05)。结论子宫内膜异位症患者卵泡液中肿瘤坏死因子的浓度升高,可能与内异症致卵细胞质量下降有关。异症可能因多种原因引起不孕症,如卵泡生成受损,尤其在中重度内异症中,低质量的卵母细胞可降低受精率,最终降低胚胎质量[3]。体外受精胚胎移植技术(IVFET)的应用对子宫内膜异位症相关不孕机制有了新的认识。
Objective To investigate the relationship between tumor necrosis factor- α in follicular fluid and outcomes of in vitro fertilization (IVF- ET) in endometriosis associated infertility. Methods The concentrations of tumor necrosis factor-α in follicular fluids from 20 patients with endometriosis associated infertility (endometriosis group) and 20 tubal factor infertile patients who undergone IVF- ET, were measured photochemically. Results No difference was observed in age, infertility period between the two groups. The required dosage of gondotrophin (Gn) was significantly more in endometriosis group than the control [ (33.40±9.48) vs (26.80±4.30), P〈 0.05]. The acquired ooeytes [ (10.8±6.87) vs (15.40±6.03)], fertilization rate [ (0.57±0.22) vs (0. 72±0.16)] and embryo amount [ (5.95±4. 32) vs (11.0±4. 22)] were significantly decreased in endometriosis group than the control (all P〈0. 05). No difference of follicle number and pregnancy rate was observed between two groups. Higher concentration of tumor necrosis factor- α in follicular fluid in endometriosis group was detected than that in control group [ (9.86±9.43) pg/ml vs (5.01 ±1.18) pg/ml, P=0. 034]. Conclusion Concentration of tumor necrosis factor - α increased in infertile patients with endometriosis, probably because tumor necrosis factor - α play a role in impairing quality of oocyte.
