摘要 目的探讨全肝动脉期多时相三维动态增强MR扫描对肝脏动、门脉系统的显示情况及其在肝脏早期强化小病灶诊断中的初步应用。方法10例有肝脏动脉期强化小病灶(直径3cm以下)的患者纳入本次研究,使用3.0T磁共振扫描仪,动脉期多时相(4期)扫描作为全肝多期三维动态增强扫描的一部分,于一次屏气下完成。绘制主动脉、门静脉及肝实性病灶的时间信号强度曲线,并采用三维最大强度投影(MIP)重建肝动脉及门静脉,记录肝动脉的显示情况并分析病灶的强化特点。结果10例患者均获得了可用于诊断的高质量图像,共发现22个动脉期强化的小病灶。采用MIP重建,4例清晰显示肝固有动脉的3级分支,4例显示4级分支,2例显示5级分支,并能显示伴行的门静脉及早期强化的小病灶。不同类型的病灶强化特点不同。结论全肝动脉期多时相三维动态增强MR扫描可同时进行肝实质动态增强MRI和MR肝血管成像,能清晰显示肝脏早期强化小病灶的动态强化过程及其与周围血管的关系。 Objective To explore the efficacy of multiarterial phase dynamic contrast-enhanced 3D MR imaging of the whole liver to depict hepatic arterial and/or portal system and its preliminary application in diagnosis of small early enhancing hepatic lesions. Methods Ten patients with small early enhancing hepatic lesions (no more than 3 cm in diameter) were included in the study. As part of multiphasic contrast-enhancement dynamic 3D MR imaging, multiarterial phase (four phases) imaging of the whole liver during a single breath-hold was performed in all patients using LAVA on a 3.0T MR unit. Signal intensity-time curves were derived from aorta, portal vein and hepatic solid lesions. The configurations of hepatic arterial and portal system were reconstructed using 3D maximum intensity projection (MIP). Manifestation of hepatic arteries and dynamic enhanced characteristics of the lesions were analyzed. Results Optimal images with diagnostic quality were obtained in all patients. Twenty-two small lesions enhanced during arterial phases were detected. On 3D MIP images, 3-order branches of proper hepatic artery were clearly visualized in 4 cases, 4-order branches in 4 cases and 5-order branches in 2 cases. Concomitant branches of portal vein and small early enhancing hepatic lesions were also well depicted. Different type of lesions showed different characteristic enhancing patterns. Conclusion Multiarterial phase dynamic contrast-enhanced 3D MR imaging of the whole liver demonstrates a useful modality to provide both dynamic imaging of liver parenchyma and hepatic angiography in a single study. The dynamic enhanced process of small early enhancing hepatic lesions and their relationships with hepatic vascular systems can be well depicted with increased spatial and temporal resolution.
